Release 11.0 – QORA ASR Gateway • Qwest will implement the ASOG Version 36 Industry Standard changes to the ASR forms and fields decided upon at an industry level with participating members of the OBF Committees. • Effective date for this release as dictated by the industry is March 17, 2008. For detailed information about the industry changes on the ASR – refer to the ATIS website: http://www.atis.org/ • Qwest will implement upfront validations in the QORA ASR Gateway that impact all ASRs being transmitted to Qwest electronically. This includes the web-based QORA (Qwest Online Request Application) GUI application, or through direct connections, such as NDM or UOM interfaces.

Customer Impacts/Value • How does implementation of Qwest up front editing add value for you, our customer? • The QORA GUI application is a superior alternative to paper ASR forms, offering on-line help as well as validations for accuracy. • Upfront edits streamline processing time for your request, by ensuring the data supplied by you is as accurate as possible. • Prior to the ASR being accepted into Qwest’s systems, you are notified of the errors on the ASR allowing corrections to be made, thereby reducing the need for costly re-issues of the ASR (supplements). • An accurate ASR helps ensure the accurate provisioning of your request the first time. • Accurate entries also create accurate billing, reducing the possibility for errors on your monthly bill. • Reduced cycle time due to less re-work and supplements being required.

General Changes • Print Functionality – QORA GUI • Qwest is implementing a change to the print functionality from Print XLS to Print PDF. • Pre-Order GUI for CFA Validation – QORA GUI • Enhancement to display the NC/NCI combination for the facility validated. • If NC/NCI is not available, a message displays “NC/NCI combination unavailable”. • ECCKT Format • Qwest is implementing a change to accept either a “/” or a “.” as the delimiter between field sections in the ECCKT field. • Copy to New – QORA GUI • Qwest is implementing a change to clear any value in the EXP field on the ASR form when the “Copy to New” functionality is used.

General Changes • User Maintenance – QORA GUI • Qwest is implementing a process to remove user access from the GUI for users that have not logged on after 1 year. • Users with submitted and/or un-submitted ASRs in the system shall be excluded. • Interconnection Translation Routing Screen – QORA GUI • Qwest is implementing a change to not provide the Interconnection Translation Routing screen when the first position of the TQ filed on the ASR form = L. • Translation Questionnaire • The system shall not require the Feature Group D section of the Translation Questionnaire when the first position of the TQ field = B.

Field Rules/Validation • RECCKT Field Validation: • Qwest will validate when REQTYP = M and the ACT = D on the ASR Form that the RECCKT is populated on the Trunking Form and ACI Form. • CFA Field Validation: • Qwest will validate when the REQTYP = M and ACT = C, the CFA/SCFA for availability. If the ECCKT is already assigned to the CFA/SCFA, the ASR will not be rejected for busy CFA/SCFA. • SECLOC Field Validation: • Qwest will validate when the REQTYP on the ASR is M or L, the SECLOC on the Trunking Form must be a valid 11 character CLLI Code. If the SECLOC is not valid, the system shall reject the ASR. • FUSF/BAN Field Validation: • Qwest will validate the FUSF exemption matches the exemption status on the BAN populated on the ASR. If a mismatch is determined, the ASR shall be rejected with error message “Invalid tax exempt status for BAN”.

Customer Responsibilities • Arrange to participate in the 30-day testing window prior to the release • Test for the ASOG 36 Industry changes per industry test plan. • Test for the new Qwest ASR Gateway upfront validations and rule edits • Ensure information on the release details is shared internally with those employees issuing ASRs to Qwest.

FAQs • Are manually transmitted ASRs impacted with this release? • Yes – the industry changes for ASOG Version 36 are applicable and the new forms must be used effective with ASRs transmitted to Qwest after 3:00 pm (MST) on Friday March 14, 2008. • Do the QORA ASR Gateway upfront edits impact our company if we have our own ordering systems? • Yes – all ASRS electronically submitted to Qwest through direct connections, e.g., NDM or UOM are impacted with the new edits/validations.
