600 Employees Say “Thank You” Thank you “I had heard about the Employee Assistance Fund. I was never sure how it worked and never thought I would qualify. Applying and receiving the help after my husband was injured and our home was burglarized was easy and helped our family receive the help we needed. Thank you to everyone who contributes to this fund.” Thank you “We had a lot of financial problems, but calling enabled us to be relieved of a lot of financial problems. They [the EAF] helped us a lot. I really didn’t know which way to turn for help. They were very kind and took a lot of pressure off. They did not judge us for asking for help. This is a wonderful program [and] they are able to help so many people. I would like to thank you once again for being there for us.” Thank you “The burden that this grant lifted from our shoulders helped us to keep seeking the best medical care for my wife so she could regain her independence after a life-threatening crisis. EAF Annual Fundraising Campaign • The Employee Assistance Fund (EAF) is part of our 150-year heritage of caring and part of what makes U.S. Bank a great place to work • Our annual fundraising campaign takes place April 1 through April 26 Please give The EAF is funded primarily by employee contributions
Take the $1.50 Challenge • In honor of U.S. Bank’s 150th anniversary, we’re asking every employee to consider pledging at least $1.50 per pay period • If you already give, consider increasing your contribution by at least $1.50 per pay period • For the cost of a candy bar, cup of coffee or can of soda, you can make a difference in the life of a colleague who’s facing a financial emergency U.S. Bank will match new or increased contributions dollar-for-dollar

About the EAF • Established in 2008, the EAF exists to help employees when they need help the most: When they’re facing unexpected or unavoidable issues that create a financial hardship • The hardships employees experience include: • Destructive storms, tornados or hurricanes • Debilitating injuries and illnesses • Deaths of family members • Military deployments that affects family income • One-time grants of up to $15,000 help cover rent or mortgage, essential utilities, food and clothing, temporary shelter and more; free legal services also are available
A Real Need, A Real Impact • Since 2008, the EAF has granted more than $2 million to over 600 employees • The need for the EAF has never been greater • In 2012, applications for assistance increased 66% over 2011 • In 2011, they increased 43% over 2010 • In 2012, 284 employees received grants from the EAF • 19 went to employees who were victims of natural disasters such as Superstorm Sandy • Of these, 16 were immediate emergency grants • Emergency grants are often distributed within hours after a storm hits
How to Contribute • By convenient payroll deduction: • Log on to Employee Self Service • Select Self Service > Payroll and Compensation > Voluntary Deductions • By check: • Mail a check payable to “U.S. Bank Employee Assistance Fund” to The Minneapolis Foundation, 800 IDS Center, 80 South Eighth Street, Minneapolis, MN 55402 • During the campaign, you also can donate points from The Best in US to make a cash contribution to the EAF, and U.S. Bank will match your contribution dollar-for-dollar • Visit The Best in US site for instructions
