  3. The Passport Difference: Value-Added Programs • Clinical teams look for opportunities to Improve Health Outcomes by: • Identifying gaps in care • Working with others to develop and plan for possible solutions • Implementing pilots and new models of care • Evaluating outcomes

Healthier Stronger Students Value Added Programs • GoNoodle Partnerships • We partner with several hospitals to offer GoNoodleto over 13,000 elementary school classrooms. • The program uses technology to get kids moving in the classroom to stimulate brain function and promote learning. • During the 2015-16 school year, Kentucky kids achieved 40,802,710 minutes of activity thanks to GoNoodle videos and Passport Health Plan. • Students that used the GoNoodle Mega Math Marathon increased their math scores by 50% more than students not using the game. • Partners are Hardin Memorial Hospital, Highlands Regional Medical Center, St. Claire Regional Medical Center, Owensboro Health, and Norton Children’s Hospital. Provider Network • Over 26,000 providers in network • More than doubled our provider network since 2014 • Some of our newest partnerships include Shriner’s Hospitals, Cincinnati Children’s, and Christ Hospitals • Over the last three years our Behavioral Health provider network has grown more than six fold *Ancillary includes providers such as Labs, Ambulatory, ASCs, and DMEs. *Additional Information on growth numbers available in Appendices A and B.

Collaborating with Our Providers • We educate our providers on migration to a new Medical Information Management System • Continuous improvement toward value-based delivery of Medicaid Managed Care • Operational changes to enhance provider experience with stronger Service Level Agreements • We hold annual workshops, roundtables, and lunch-and-learns • We keep providers informed on pertinent changes and announcements via eNews • We provide meaningful data-driven reports

Health Education: LIFE Program Passport Health Plan has a health educator who provides healthy lifestyle trainings free of charge to our members and the community at large through the Lifestyle Improvements for Everyone (LIFE) Program! Topics include: • Nutrition and exercise • Life skills and hygiene • Stress and coping • Disease management • Bullying prevention
