OHIO University Student Health Insurance Policy • All students are required to have an active health insurance policy while attending OHIO University • Your health insurance is provided by a commercial company named United Healthcare Student Resources. • Your insurance is valid July 26, 2015 through August 15, 2015

  4. INSURANCE ID CARDS • Insurance Identification Cards are provided to you in your insurance information packet • You will need your identification card to: • Receive Medical Treatment • Purchase medications from a pharmacy at a reduced rate

  5. WHAT DOES THE INSURANCE PAY FOR? The health insurance benefits at Campus Care are for: • An office visit to see a doctor if you are sick or injured • You will owe a $15.00 copay to see a doctor at Campus Care • Laboratory testing for diagnostic purposes • X-Rays (Radiology) • Reduced rate for medications from the Campus Care pharmacy • You will owe $5.00-$35.00 for medications purchased at the Campus Care Pharmacy

  6. WHAT DOES THE INSURANCE NOT PAY FOR? The Health insurance will NOT pay for: • An office visit to see a doctor when you are not sick, or do not have an injury or illness • The $15.00 copay to see a doctor at Campus Care • Dental Treatment • Vision Screenings • Medical treatment for items listed on pages 3-4 of the policy summary; PLEASE REVIEW

CAMPUS CARE SERVICES Campus Care provides the following services to all students: • Eating Disorder Clinic • ADD/ADHD • Allergy Shot Clinic • Immunizations • Tuberculosis Testing • Counseling Services • Psychological Services • Transgender Health • Emergency Contraception • Walk-In Clinic Hours=No Appointment Necessary • Sexually Transmitted Infections • HIV Testing • Asthma • Laboratory Services • Radiology (X-Ray) • Physical Therapy • Pharmacy • Women’s Health Clinic

  9. What if Campus Care is Closed? Patient will be responsible for part of the medical expenses when treated outside of Campus Care. Create Your Account with United Healthcare All students enrolled in the student insurance plan can set up an on-line account with United Healthcare Student Resources. • You will need your Ohio.edu email address and Insurance SRID Number from the front of your ID card • Log onto www.uhcsr.com/MyAccount • Follow the directions to choose your username and password

United Healthcare Website: www.uhcsr.com/Ohio Upon creating an online account with United Healthcare Student Resources (www.uhcsr.com/Ohio ) students will be able to: • Print a copy of ID card/Upload digital copy • Search for providers and facilities In-Network if you are more than 30 miles away from Athens • Access policy information • View Explanation of Benefits • Access claim letters • Complete Accident Details form • Access additional forms needed • Update personal information

Create online account with the insurance company (www.uhcsr.com/MyAccount) to print or upload a new ID card (Free) • Locate Network Provider: Visit www.uhcsr.com/Ohio and select UHC Choice Plus Network • Additional Information: Available on the Bursar’s website: http://www.ohio.edu/finance/bursar/studenthealth.cfm
