PRESENTACION DE CASO DATOS PERSONALES Sexo femenino 22 años de edad Obesa Morbida ANTECEDENTES CEVLP por colecistitis aguda litiasica ( 1-4-09) Pancreatitis aguda litiasica grave post colecistectomía (al 7mo dpo) SERVICIO DE CIRUGIA HTAL. MOTIVO DE CONSULTA Ingresa a nuestro servicio derivada de otro centro a las 5 semanas de post operatorio por : Dolor Abdominal – Ictericia - Vómitos LABORATORIO GB 9500 HCT 30 % HGB 12 mg AMILASA 130 U FAL 890 U ASAT 40 UI ALAT 35 UI BT 5 mg BD 3 mg SERVICIO DE CIRUGIA HTAL. ECOGRAFIA HIGADO : sin particularidades COLECISTECTOMIZADA VBIH : no dilatada VBEH 9mm con ecorrefringencias distales compatibles con litiasis Se observa colección liquida homogénea, redondeada en proyección cefalo-pancreatica de 90 por 45 mm SERVICIO DE CIRUGIA HTAL.   8. CPRE • Abombamiento parietal a nivel de antro en su cara posterior y compresión extrínseca de bulbo. • VB de 9mm con imágenes de lito en su interior. Papilotomia, extracción de 2 litos. DEFINICION • Toda colección localizada de secreción pancreática delimitada por tejido de granulación no epitelizado, con variable cantidad de tejido necrótico o no y con valores altos de amilasa, generado en respuesta a la disrupción del sistema canalicular. • Complicación mas frecuente de la pancreatitis aguda o crónica. • Representan el 75% de las lesiones quisticas del páncreas. PSEUDOQUISTE AGUDO - TRATAMIENTO N : 41 casos de 570 pancreatitis tratadas entre enero 1998 – Agosto 2010 Edad 16 – 76 años ( media 48) Sexo femenino predominantemente ( 63%) Sexo femenino predominantemente ( 63%) SERVICIO DE CIRUGIA HTAL.   34. COMPLICACIONES 13 PROCEDIMIENTOS Morbilidad : 15.3 % 2 casos de quistogastrostomia : infección Tratamiento endoscópico SERVICIO DE CIRUGIA HTAL. ITURRASPE Cuando la cavidad quística contiene elementos sólidos, es útil la colocación de un drenaje nasoquistico para prevenir la infección mediante lavados. CONCLUSIONES • Aproximadamente 50% de los pseudoquistes pancreaticos regresan espontaneamente y no requieren tratamiento • Si el tratamiento esta indicado, las técnicas miniinvasivas ( laparoscopicas , endoscopicas y percutaneas) en casos bien seleccionados tendrian mejores resultados en cuanto a morbilidad, mortalidad, y confort para el paciente. • La técnica endoscopica debe realizarse en centros especializados, en manos de endoscopitas entrenados, quienes trabajen en equipos multidisciplinarios capaces de resolver cualquier tipo de contingencia SERVICIO DE CIRUGIA HTAL.
