  1. New Hire BasicMED Plan • A short-term medical plan available July 1, 2009 through June 30, 2010 • If you have been hired in the Alpine, Davis, Granite, or Jordan School District and are without health insurance coverage for the first 60-90 days of your employment, the Utah Education Association and its affiliates will pay 80% of the cost of this New Hire BasicMED Plan administered by Educators Mutual Insurance Association (EMIA). • UEA membership required.

  2. What does New Hire BasicMED cost? • Premiums for 2009-2010 increased by $0.03 (to round to the dollar). * This benefit is only available to new UEA members in the Granite District.

  3. What does New Hire BasicMED cover? • Medical benefits • Up to $10,000 • Provides bridge to employer plan • Non-panel benefits available in 2009 • Prescription benefits • Plan will pay up to $5 per prescription • Up to 80% discount savings

  4. When does coverage begin? Granite – Effective date of group coverage is 90 days following start day. Traditional Schools: • 30-day enrollment window for insurance paperwork is Sept. 17, 2009 • 90-day waiting period begins on Aug. 18, 2009 and ends on Nov. 16, 2009 Four-Track Year Round Schools: • 30-day enrollment window for insurance paperwork is Aug. 26, 2009 • 90-day waiting period begins on July 27, 2009 and ends on Oct. 25, 2009 Single-Track Year Round Schools: • 30-day enrollment window for insurance paperwork is Aug. 28, 2009 • 90-day waiting period begins on July 28, 2009 and ends on Oct. 27, 2009

  5. Has coverage changed? • Changes in 2009 • Non-panel benefits are available. • Changes in 2008 • Preventive services are available with a $20 co-pay without being subject to the deductible. • Child immunizations are covered for children under the age of 2. • Plan will pay up to $5 per prescription.

  6. What is the enrollment process? • UEA and the Granite Education Association will receive four items: • A UEA Legal Agreement (specifying that those who do not remain members of the Association for a minimum of one year will be required to pay back the cost of the benefit) • A New Hire BasicMED Brochure • Plan Highlights (including costs) • A New Hire BasicMED Enrollment Form

Enrollment continued… • UEA and Granite recruiters MUST: • Ask new members to sign both the Enrollment Form and the UEA Legal Agreement. • Return the Enrollment Form, Legal Agreement and copy of the Membership Form to the UEA Membership Department ON THE DAY THEY ARE RECEIVED. Enrollment continued… • Membership will be verified by the UEA membership Department before the forms are delivered to EMIA. • The UEA Membership Department will confirm that all three pieces are properly completed and signed in the appropriate places. • If forms have not been completed correctly the UEA Membership Department will contact the UniServ office for follow-up.

  9. Enrollment via phone or web! • Members may call EMIA at (801) 262-7475 or (800) 662-5851 • EMIA will verify membership with the UEA Membership Department. Enrollees may pay by credit card or ACH or may pay by check if paid in full. EMIA will send copies of the enrollment form and UEA legal agreement to the UEA Membership Department. • Members may visit the UEA website for information • New members will need to look for the UEA “New Educator” logo at www.utea.org and then enter the number on the front of their New Educator Membership Card. • They will be taken to a second page where they will see an EMIA logo. Instructions for enrollment will be included.

  10. Confirmation Letter • EMIA will send a letter of confirmation to those who sign up. These letters will be created by the local associations, on their letterhead. • The letter will be sent only after the enrollment process has been completed, and will let the member know that the policy is in effect. • The letter will be sent electronically if possible. • EMIA will CC UEA and/or the local uniserv on the letters.  • EMIA will remind members in the email that this is not the district coverage.

Questions? • When do rates change? • Annually • How does this plan impact waiting periods; does HIPAA apply? • This plan can shorten pre-existing periods on district coverage • HIPAA applies • Will there be COBRA coverage? • Yes, but only for 60-90 days (not for 18 months, per COBRA law on short term products)
