  3. K-HEN VTE Info • 15 hospitals committed to project • 3 hospitals have entered baseline data (20%) • 4 hospitals have entered monitoring data (27%) • 4 - Progress reports submitted for July and August

  4. VTE Measures K-HEN recommended measures • Outcome: Number of surgical patients that develop a post-operative PE or DVT (#104) OR Potentially preventable VTE (#103) • Process: VTE patients receiving unfractionated heparin with dosages/platelet count monitoring by nomogram or protocol (#100) OR VTE Discharge Instructions for patients with confirmed VTE (#99)
