  4. The proper coding of answer documents is most easily accomplished one test administration at a time by following the detailed and specific instructions provided in your manuals. While some of us are out of our comfort zones reading the directions first, it is advantageous to do so. Coding of 2011 TAKS/TAKS-M Answer Documents

Why does it matter?!? • After an answer document has been submitted for scoring, districts will NOT be able to make corrections to the student information such as demographic, program participation or score code status. • Inaccurate student information may prevent the current test administration data from merging with a student’s permanent record file. • District/campus accountability could be adversely affected. • A student’s document might not be scored. So How Do I Get Started? • Upon receipt, the preprinted information on all answer documents and precoded labels must be verified for accuracy and completeness. • The information submitted on the answer document should reflect the student’s status at the time of testing. • If the information has been precoded correctly, there is no need for it to be hand-gridded on the answer document. • Hand-gridded information will override any precoded information other than the student name, PEIMS number, date of birth and gender. Demographic or Program Information • If a demographic or program information field is left blank for any student, data from the October 29, 2010 submission will be used to complete the field if possible. • If it is not possible to match a student’s scorable document to the appropriate PEIMS record, data from any field left blank will be aggregated and reported as “No information provided.” Coding of 2011 TAKS/TAKS-M Answer Documents

Correcting Precoded Errors • To correct an inaccurate student name, PEIMS ID, or date of birth on an answer document: • Void the document by writing “VOID” in large, bold letters across the student information grids. • Print and hand-grid the correct information on a blank answer document with a No. 2 pencil. • Place the voided answer document under a Voided Answer Document Identification Sheet and return with scorable materials. Correcting Precoded Errors • To correct an inaccurate student name, PEIMS ID, or date of birth on a label: • Write “VOID” across the label to return with nonscorable materials. • If the label has already been affixed to a scorable document write “VOID” across the student information grids on the document to return with scorable materials. • Print and hand-grid the correct information on a blank answer document with a No. 2 pencil. Score Code Field • The SCORE CODE field in the FOR SCHOOL USE ONLY section indicates whether a student’s test should be scored. • For each subject area test, select only one score code. • Failure to mark the appropriate score code may result in a score of ZERO for the student. Test Taken Info • The TEST TAKEN INFO field must be correctly gridded to indicate which test, or combination of tests, a student was administered in order for the test to be scored accurately. • Note: Grade 3 scorable test booklets do not have this field.
