HSE LEARNINGS. INCIDENT: Twenty four Galfar employees out of 144 residents in TAC, Nimr reported to Galfar clinic with symptoms of abdominal pain and diarrhoea. The were treated with diarrhoea tablets and ORS powder. The symptoms subsided within nine hours. Root Causes:

  1. HSE LEARNINGS • INCIDENT: • Twenty four Galfar employees out of 144 residents in TAC, Nimr reported to Galfar clinic with symptoms of abdominal pain and diarrhoea. The were treated with diarrhoea tablets and ORS powder. The symptoms subsided within nine hours. • Root Causes: • Biological infection from food resulting in mild poisoning • Inadequate frequency of internal inspections and audits of HACCP. Quarterly and annual inspections and audits are not effective • Hygiene maintenance of camp facilities is not supervised for quality and effectiveness • LEARNING: • Establish an assurance mechanism for compliance to HACCP (Codex) and Hygiene standards • Key Learning: • Check the expiry dates on foodstuff purchase and consumption. • Store food as per instructions • All Medical Incidents must be reported to PDO clinic Immediately OHI – Food Poisoning Use this Alert: Discuss in Tool Box Talks and HSE Meetings  Distribute to contractors  Post on HSE Notice Boards  Include in site HSE Induction Contact: MSE511 for further information or visit the HSE Website Alert No 69 July, 2011
