Job Description Review& Approval: 12/28/2006 HRReview: GDC Recommended: Personnel CommitteeApproval: Job Title: UPN: SOC / DOTCode: Division: Section: Location: Shift: Reports To: Grade/Step: SalaryLevel: FLSAStatus: InitialApproval: UpdatedOn: Dietitian SVP-01 29-1031 / 359367014 CommunityServices NutritionServices Potawot HealthVillage, Arcata Monday - Friday, 8:00 a.m. to 5:00 p.m. Maternal & Child Nutrition Health Supervisor 10-01 to 12-10 $14.84 to 424.22perhour,or$30,867to$50,378peryear Exempt–Salaried November15,2001 Changes 10/14/05, & 12/28/2006 • SUMMARY • The Dieticianisresponsibleforthe medicalnutritiontherapycareofUIHSclientsandworkingonefforts topreventdiseasethroughnutrition education. • ESSENTIALDUTIESANDRESPONSIBILITIESincludethefollowing. • Interviews Clients to obtaininformationregardingdiet,life stylehabits,culture,andphysical activity. • Formulates goals for diet/mealplanswithclients tailoredtotheirindividualneeds. • Counsels clients to promote goals andevaluatesprogresswithclients. • Promotes health throughnutritionforclients throughoutthelifespanusingaculturallysensitive approach. • Provides nutrition educationfortheAmericanIndiancommunityandgroups. • Provides outreach to othercommunityagencies orgroups. • Document contacts in theclienthealthrecord. • Attend department meetingsandotherassignedmeetings. • Adheres to accreditationandcompliancestandards/guidelines. • Maintains a professional,organized,andcleanworkingenvironmentby followingorganizational policies,guidelines,and safetystandards. • Assists indevelopmentofdepartmentalpolicy,procedures,andqualityimprovementactivities. • Other duties may be assigned

Level II (Dietician II) performsalloftheEssentialDutiesAndResponsibilitieslisted above,andassists withdevelopmentofnutritioneducationmaterial. • Level III (Dietician III) performsalloftheEssentialDutiesAndResponsibilitieslisted above,and Assistswiththedesignandimplementationofnutritioncareplansforclientswithdiagnosedmedical conditions. • SUPERVISORYRESPONSIBILITIES: • Level I hasnosupervisoryresponsibilities. • Level II & III supervises NutritionAssistant(s) in theabsence ofMaternalandChildNutrition Supervisor. • QUALIFICATIONSToperformthisjobsuccessfully,anindividualmustbeabletoperformeach essentialdutysatisfactorily.Therequirementslistedbelowarerepresentativeoftheknowledge,skill, and/orabilityrequired.Reasonableaccommodationsmaybemadetoenableindividualswith disabilitiestoperformtheessentialfunctions. • EDUCATION and/or EXPERIENCE: • Level I & II: BachelorsDegree. • Level III: Bachelor’sDegreeandinaddition,musthaveadvancedprofessionalcertification whichpermitsnutritiontherapyinspecializedcategories. • LANGUAGESKILLS: • Ability to read, analyze, and interpretcommonscientificand technical journals,financialreports, andlegaldocuments. • Ability to respond to common inquiries or complaintsfromcustomers,regulatoryagencies,or membersofthebusinesscommunity. • Ability to write speeches and articles forpublicationthatconformtoprescribedstyleandformat. • Ability to effectively present information to top management, public groups, and/or boardsof directors. • MATHEMATICAL SKILLS: • Ability to work with mathematical conceptssuchasprobabilityandstatisticalinference,and fundamentalsofplaneandsolidgeometryandtrigonometry. • Ability to apply concepts such as fractions,percentages,ratios,andproportionstopractical situations. • REASONINGABILITY: • Ability to define problems, collectdata,establish facts,anddrawvalidconclusions. • Ability to interpretanextensivevarietyoftechnicalinstructionsinmathematicalordiagramform anddealwithseveralabstractandconcretevariables. • CERTIFICATES, LICENSES, REGISTRATIONS: • Level I : Must obtain aRegistered Dietitiancertificationwithinoneyearofhire. • Level II & III: MusthaveRegistered Dietitiancertification. • All Levels:Mustpossessavaliddriverslicense andbeable tobecoveredbyagency's insuranceprogram;mustprovideproofofprivatevehicleinsurance;mustpossessor beableto obtainCPRcertification withinsixmonthsofhire andmaintainsuch. • PHYSICALDEMANDS:Thephysicaldemandsdescribedherearerepresentativeofthosethat mustbe metbyanemployeetosuccessfully performtheessentialfunctionsofthisjob.Reasonable accommodationsmaybemadetoenableindividualswithdisabilitiestoperformtheessential functions.
