There was a LV Cable Strike o/s of 5 Eastcote Rd, Pinner on Friday 10/10/14 at approx. 14:56.The original Excavation was excavated by another team.The Lanes team on site required the excavation the be extended to allow them room to insert a lining into a storm drain.A second team was sent to carry out this extra works.

  3. The team discovered a 150mm Gully pipe running into the Storm drain with concrete around it.

  4. In the process of removing the concrete from around the Gully Pipe, with an FL22, they struck the LV cable below. LV Cable

  5. What Went Wrong • The Team did not carryout a site survey using the CAT4 & Genny because the LV cable had been “Marked Up” by the previous team who excavated the trench.

  6. The Team did not use the use the CAT4 to trace out below the Cable TV Ducts. • The Team did not Hand dig to Locate the LV Cable before trying to remove the concrete.

  7. Lessons Learn’t • Always use the CAT4 & Genny to survey the site when taking over a Job, started by others • Never rely on other peoples Marking Up. • When excavating Always Scan below Exposed Cables/Plant. • Before knocking out concrete in the ground, Always hand dig 500mm around the concrete to ensure that there are no cables/plant in the immediate • Supervisors are to ensure there is a suitable and sufficient handover and exchange of information on site where the gangs change
