ECAC LETTEREC 9/14.37 - 0032 Air Passenger Health Issues Working Group (APHING) Second Meeting – Paris, October 2003 • PROPOSALS FOR: • Revised content of first aid training for cabin crew • Revised content of First-Aid Kits • Revised content of Emergency Medical Kit • Introducing 2 new concepts: the “optional equipment” based on “risk assessment” of the airline • example of “optional equipment”: intubation set/IV fluids/automated external defibrillator (AED)

ASSESSMENT OF PROPOSALS – METHOD USED: • The ECAC proposals were compared to: • JAR-OPS 1 requirements/advisory material; • Latest JAR-FCL according to NPA 37; • ICAO Doc 7192 AN 857, Part 1, Training Manual – Cabin Attendants’ Safety Training, Chapter 8 – “Hygiene, aviation medicine and first aid”. (Details in this Manual are not all-inclusive, and are meant as guideline to the minimum requirement for the training of cabin crew). • Missing/exceeding items in the sources compared, were listed. • Benefits of the proposals were assessed in light of the Agenda for Change. • Conclusions were drawn on the operational impact of these proposals. • Cooperation with Licensing (M) was sought. Their suggestions to the next LSST (M), in relation to the ECAC proposals were taken into consideration in this analysis (see attachment 1 – FAKEMAK)

  4. NOVELTY IN THE ECAC PROPOSALS: 1. Cabin Crew Training in First Aid: • Introduction of guideline on Travel Health into the IEM to Appendix 1 to JAR-OPS 1.1005/1015/1020 – First Aid Proposed sub-chapter on “Travel Health”: • Infectious diseases; reporting infectious diseases; protection from infection; clinical waste; avoiding water/food-borne illnesses • Aircraft disinfection • Alertness management: Fatigue, Sleep physiology; Circadian rhythm; Jet-leg To be decided: - If to amend the above IEM, to include this proposal.

NOVELTY IN THE ECAC PROPOSALS • Content of First-Aid Kit: (see ECAC Attachment 1; see Attachment 1 FAKEMAK – Licensing (M)) • Introduce into the First-Aid Kit: • safety pins (not existing in AMC OPS 1.745, but present in the latest JAR-FCL according to NPA 37) • biohazard disposal bags (not in AMC OPS; not in JAR-FCL/NPA 37) • medical incident report form (not in AMC OPS; not in JAR-FCL/NPA 37) These proposals will be considered by the next LSST (M), in March, 2004. • The First Aid Handbook may not necessarily be placed in the First Aid Kit, although present on board. To be decided: - The inclusion of the items listed, into the First Aid Kits. - What is the impact of not placing the First Aid Handbook into the First Aid Kit, onto the existing standard procedures, and decide if the proposal is substantiated.

NOVELTY IN THE ECAC PROPOSALS 3.Content of Emergency Medical Kit and Optional Equipment • Items proposed by ECAC that are missing in AMC OPS 1.755 – Emergency Medical Kit: • basic delivery kit (proposal exists in the latest JAR-FCL, according to NPA 37) • tweezers (not in JAR-FCL/NPA 37) • bag valve mask (not in JAR-FCL/NPA 37) • blood glucose testing equipment (not in JAR-FCL/NPA 37) • thermometer – non mercury (not in JAR-FCL/NPA 37) • aspirator (not in JAR-FCL/NPA 37) • intubation set (not in JAR-FCL/NPA 37) • IV Fluids (not in JAR-FCL/NPA 37) • automated external defibrillator (AED), (not in JAR-FCL/NPA)

  7. NOVELTY IN THE ECAC PROPOSALS • It is proposed that items 7 – 9 (“optional equipment”) be considered according to the conclusions of the risk assessment carried out by the airline. Proposals will be presented to the next LSST (M) – in March, 2004. To be determined: • if AMC OPS 1.755 should be amended to make reference to items 1 – 9. • whether to introduce the concept of optional equipment (7 – 9), to be carried on board, based on the risk assessment conducted by the airline.

  8. CONCLUSIONS: • Decide whether to amend IEM to Appendix 1 to JAR-OPS 1.1005/1015/1020 – First Aid, to contain reference to Travel Health. 2. Revised content of First-Aid/Emergency Medical Kits - subject to consideration of LSST (M). 3. Decide on the introduction of the concepts of “optional equipment” and “risk assessment” in relation to equipment.

CONCLUSIONS: OPERATIONAL BENEFITS OF THE ECAC PROPOSALS: • Inclusion of Travel Health into the cabin crew first aid training • improved first aid performance of cabin crew • better knowledge of self-care • Information on optional equipment based on risk assessment • may prove to be useful to airlines, mainly • those just starting operation
