  3. Physical Informationwhite sheet All physicals are due by the End of May. I must have a copy of the actual physical returned in this folder by May 24.

  4. Parent Informationpink sheet Emergency Contact Team Parents and volunteer’s Will need EVERYONE’S help, but, if you’re willing to be a “main” helper, check the ‘yes’ box!  Help with snack schedules Help with organizing, setting up, and doing fundraiser events

  5. Cheerleading Agreementyellow sheet Cheerleading is a HUGE time commitment! Attending practices, games, events, etc. is REQUIRED of ALL cheerleaders! Demerits will be handed out! We will be much harder on absences this year! We understand that things come up, but if attendance becomes an issue, your placement on the squad will be evaluated. We ask that you make medical appointments outside of cheerleading, if at all possible! REQUIRES PARENT SIGNATURES!!

Plan C: 4 payments of $93.75 1st payment due April 10th 2nd payment due May 1st 3rd payment due June 1st 4th payment due July 1st Warm-ups: Purchase: $65 order by August 1 Rent: issued in August NO REFUNDS!! Once order and commitment has been made, means payment is due! No exceptions. Payment Plans (Continued)

  9. Voucher(white sheet) This is a way for you and I to keep up with how much you owe. This includes major due dates and for the items they belong to. For example, uniform rental fee, warm-ups if purchased, and camp gear If payment of $93.75 is not in by 4/10, then they can not cheer.

Forms • All forms are due May 24th • These can be turned in to Ms. Love. • They can also be dropped off in the front office of MMS. • Parent signature required on: • Cheerleading Permission Slip • Cheerleader Agreement • Payment Plans • Rental Agreement • Physical • Please turn all forms in with the file folder!

  11. Expectations 1. Proper workout attire will be worn during all practices/workouts! Appropriate shorts/shirts Hair ALL THE WAY back NO JEWELRY!! 2. Respect will be given at all events (workouts, practices, games, etc). To the sponsor To each other To the school!

  12. Responsibility The girls need to follow all the rules and guidelines in the cheer code of conduct. They are leaders of the school so we hold them to a high standard. 8th grade is a year for the girls to learn so I allow them to be responsible for their schedules and commitments

  14. Time Commitment 1.Cheer Season - MAY 2013 to May 2014 - Camp Practice May 7, 14, and 21 at McMath 7:05 am Gold Gym -Camp: August 6,7, and 8th at Harpool MS 2. Practice: TBD - Option 1 7:05 to 8:05 am Wednesday - Option 2 Friday afternoons until 5:30 3. Games - One night a week - 5:15 pm to 9 at the latest Football Basketball (tentatively)

  15. Time Commitment 4. Pep Rallies - we have two evening practices to prepare for the pep rallies and/or extra practices as needed 5. Transportation: -Parents are responsible for transporting their children to all cheer events. -Must be on-time for games and on-time to pick up your cheerleader

  16. Dates In addition to the payment plan due dates Due May 24th 1. Physical 2. Shoes *Physical – Valid for 2 years Camp is Aug. 6-8 at Harpool MS (attendence is required!)

  17. Good Examples Shoes must be all white. Cheer shoes should have a smooth sole.

  18. What not to get: These have too much black and the bottom is too rough. Cheer shoes have smoother soles.

  19. Suggestions of Sack Lunches for Camp Camp Lunch Ideas: 1. Main item Sub sandwiches Chicken sandwiches Pizza 2. Sides Fruits Yogurt Veggies & dip

  20. Donations Camp: Small bags of candy Recycled or new tissue paper Baked goods for other squads

  21. Dance Donations Dance Needs: Candy Chips Sodas Pizzas Prizes VOLUNTEERS *The dance is the first Friday of the school year (August 30).

  22. Parent Shirts • $10 • Navy Shirt • All sizes • McMath Cheer Parent • Please order by May 1st if you would like to wear yours for camp.
