Any employee can raise a hazard or safety related request by logging on http://flightsafety.airindia.in and going in to online reporting section • SMS Process workflow has automate various reporting forms that are available on flight safety website • It will be helpful for safety management system (SMS) team to track and monitor various hazard reports submitted by employees • Various DMS workflow processes will be used for reporting different report: SMS Reporting • Anonymous reporting • Confidential reporting • Voluntary reporting Mandatory Reporting • BIRD STRIKE reporting • EGPWS reporting • FDTL reporting • FSR – AI reporting • FSR-IX reporting • TA Reporting • Any employee can raise a hazard or safety related request by logging on http://flightsafety.airindia.in and going in to online reporting section AI Ltd. SMS Anonymous Reporting Form 7 2 9 3 1 4 5 6 8 4: Select multi Dept. check box 1: Select AOP / SBU Name which will receive this request for action 2: Unique request number 3: Select this button in case single Dept. is needed 8: This section will display attached documents 7: Add supporting document for reporting 5: Select this button in case multi Dept. is needed 6: Hazard / Event related fields /details 9: Select action and click on submit to send it for processing AI Ltd. Mandatory Reporting • Following mandatory reporting can be raised :- • BIRD STRIKE reporting • EGPWS reporting • FDTL reporting • FSR – AI reporting • FSR-IX reporting • TA Reporting • All the mandatory reporting will have same action and feature like SMS reporting • It can be raised in same way as described in earlier slides for SMS reporting AI Ltd.
