Chief Complaint • Upper anterior teeth – “they are getting more loose everyday” • “My dentist told me I have gum disease and that I need gum surgery and implants”
Medical History • General Health: Patent is in good overall health • Allergies: None • Medications: None • Smoking: None
Periodontal Examination • Probing depths: 3-9mm pockets • Mobility: #8, #9 (Grade 3), #10 (Grade 2), and #7 (Grade 1) • Recession: Maxillary anteriors:#6, #7, #8, #9, #10, and #11
Diagnosis • Periodontal Diagnosis: Generalized moderate chronic periodontitis and localized severe chronic peridontitis With bleeding on probing (Patient may have had aggressive periodontitis when he was younger)
Patient expectations • He would like to address the pockets in the posterior teeth and then address the upper maxillary anterior teeth • He would like to have implants and “something fixed”
