  2. Read each statement and select the appropriate number response as it applies to you. Often = 3 Sometimes = 2 Seldom/Never = 1

  3. 1. I remember information better if I write it down.

  4. 2. Looking at the person helps keeps me focused.

  5. 3. My papers and notebooks seem messy.

  6. 4. When I read, I use my index finger to track my place on the line.

  7. 5. I start a project before reading the directions.

  8. 6. I hate to sit at a desk for long periods of time.

  9. 7.I need a quiet place to get my work.

  10. 8. When I take a test, I can see the textbook page in my head.

  11. 9. I do not follow written directions well.

  12. 10. If I hear something, I will remember it.

  13. 11. I prefer first to see something done then to do it myself

  14. 12. I use the trial and error approach to problem solving.

  33. For Visual Modality compute your answers in item numbers: 1,2,7,8,13,14,19,20,25 and 26.

  34. For Auditory Modality compute your answers in item numbers: 3,4,9,10,15,16,21,22, 27 and 28

  35. For Kinesthetic/Tactile Modality compute your answers in item numbers: 5,6,11,12,17,18,23,24,29 and 30.

  36. The highest of the three scores indicates the most efficient method of information intake.

  37. The second highest score indicates the modality which boosts the primary strength.

  43. Auditory:1. Talks to self aloud.2. Enjoys talking.3. Easily distracted.4. Has difficulty with written directions.

  44. 5. Likes to be read to.6. Memorizes sequentially.7. Enjoys music.8. Whispers to self while reading.9. Distracted by noise.

  45. 10. Hums or sings.11. Outgoing by nature.12. Enjoys listening activities.

  46. Kinesthetic1. Likes physical rewards.2. In motion most of the time.3. Likes to touch people when talking.

  47. 4. Taps pencil or foot when studying.5. Enjoys doing activities.6. Reading is not a priority.7. Poor speller.
