Rehires • Rehire Rule • Employees who return within 30 days and within the same plan year • Considered a “rehire” • Coverage is reinstated with no changes • Employees who return after 30 days or in a new plan year • Considered a “new hire” • Receive Hewitt enrollment information and enroll as a new hire • May select different coverage

  4. 16th of the Month Rule for Terminations • Terminations before the 16th • Coverage ends the last day of that month (excluding STD & LTD) • Payroll deductions end effective the first of that month • Example: Termed June 1st; Coverage ends June 30th Terminations on or after the 16th • Coverage ends the last day of the following month (excluding STD & LTD) • Payroll deductions end effective the first of the following month • Example: Termed June 16th; Coverage ends July 31st

Terminations for Summer paid BOE employees • Coverage for employees will end based on the termination date entered by the benefit professional • Termination date of June 1st entered; Coverage ends June 30th • If an employee is receiving a paycheck through the end of August, the termination date should reflect this • Example: Employee’s last paycheck is August 31st; Termination date entered should be September 1st; Coverage will end September 30th

  6. 16th of the Month Rule (Status or Coverage Changes) • Status changes are driven by employment data • Leaves, Terminations, Retirement, etc. For Status Changes • Before the 16th of the Month • Coverage change/end effective the first day of the next month • (Example: Change on June 1st; Effective July 1st) • On or after the 16th of the Month • Coverage change/end effective the first day of the following month • (Example: Change on June 16th; Effective August 1st)

  7. 16th of the Month Rule (Status or Coverage Changes) • Qualified Status Changes are initiated by employees • Birth, Adoption, Marriage, Divorce, etc. For Qualified Status Changes • Effective the first of the month following the date the change is reported (regardless of actual QSC date) • Birth and adoption are effective on the actual date of the event • Employees report a QSC to GaBreeze • Supporting documentation issued to SPA • Within 30 days of the event to request changes

Dependent Information • Dependent Information on File • Employees must contact the GaBreeze Benefits Center to update dependent data • Dental and/or Vision carriers will not accept dependent information • Dependent coverage remains in place through 2010 for participants with missing dependent information • Employees will be required to updated dependent data when a QSC results in Benefit changes or enrollment • Dependent enrollment will be required for Plan Year 2011 for the upcoming Annual Enrollment

Changing Agencies (for Employees) • Employees who Change Agencies • Hewitt receives a termination notice from the former agency before receiving eligibility information from the new agency • GaBreeze will terminate coverage and mail COBRA notices • Once eligibility information is received, GaBreeze will remove COBRA data and coverage will continue with the new agency • Hewitt receives eligibility information from the new agency before receiving termination notice from the former agency • New agency status will be “Pending” until termination notice is received • Once termination notice is received, status will be updated and coverage will continue • Timely action of the Benefit Professional is critical to ensure continued coverage

  10. Multiple Agencies (for Employees) • Employees who work for Multiple Agencies • The Primary Agency will be the employee’s first agency of employment by recorded hire date • New hire employees will not have a choice • Selection for current employees will remain the primary agency

Retirement • Employees who Retire • Benefit Professional enters a retirement status • Hewitt will mail a Confirmation Kit • Retiree Dental information and COBRA information (Vision and Health Care FSA) if currently enrolled • Retiree Dental is automatic; no enrollment form required • Retiree can enroll in COBRA Vision and/or Health Care FSA • Hewitt will direct bill participant and send any premium updates • Contact Hewitt to make option/coverage changes • The Confirmation Kit also includes phone numbers to the other vendors for switching coverage to an individual policy
