  6. What is meant by intellectual and developmental disabilities? Intellectual (manifest before 18) IQ - Medical Model Cognitive Ability - School Model Processing, understanding, and comprehension Adaptive Behavior/functioning Daily functioning Social Skills Learning Developmental Disability (manifest before 22) Physical Impairments cerebral palsy, epilepsy, autism, or other disabling conditions (may or may not contribute to intellectual problems) Down Syndrome, fetal alcohol syndrome, and fragile X syndrome (may co-occur with intellectual problems) Blindness, Visual Impairments, Hearing Impairments, etc. (may or may not contribute to intellectual problems)

Purpose of the SIS: Assess support needs through SIS Assessment Determine the intensity of needed supports through SIS Results Monitor progress through a review of prior SIS scores, plot progress Evaluate outcomes of Adults with IDD Are the correct services needed being provided based on the intensity of support and needs

Rating Scale SIS Ratings Are determined without regard to the services or supports that are currently being provided Reflect supports that would be necessary for the person to be successful when engaged in a particular activity If a person uses augmentation equipment the person is rated with the said technology in place All items are completed by the SIS Practitioner even if the person is not currently performing a listed activity in order to determine an individual’s overall intensity of support needs Takes into consideration supports needed for participation at a later time

  15. How to Schedule a SIS Examination in North Carolina

Contact a Licensed Independent SIS Practitioner Obtain a copy of the Practitioner’s monthly availability Check to make sure that the person being evaluated has both Medicaid and Medicare, but no third party insurance Schedule the SIS Assessment with family Talk about the advantages of having a SIS completed Provide family with a sample copy of the interview form Make sure at least two respondents are present for the interview Participant should serve as one of the respondents if at all possible Obtain consent form for Child SIS

  17. Interpreting the SIS Results to Assist in Person-Centered Planning

Reviewing the SIS Interview and Profile Form (Demographics) Contain pertinent Information Field Rating Scale for the Six Activity Life Domains Type of Support – what type of support is needed for the person to engage in the life activity Frequency – how frequently is or would the support be needed if the individual engages in the life activity Daily Support Time – how much daily support time is or would be needed for the person to engage in the life activity

Reviewing the SIS Section 1B: Support Needs Profile The higher the support needs index score the greater the intensity of support needs Provides a visual graph or pattern of a person’s support needs across the life activity domains Normed on individuals with intellectual and developmental disabilities Assist in prioritizing domains by standard score The higher the standard score the greater the intensity of support needs in that Domain

Breaking task domains down for person-centered planning Consider what is most important “to” and “for” the individual (Refer to SIS Practitioner’s report) Prioritize highest domain areas (standard scores) Identify tasks within those domains requiring highest intensity of support and frequency Identify items requiring extensive medical and behavioral support needs Review health and safety domain tasks Review protection and advocacy ranking

Breaking task domains down for person-centered planning Consider pertinent information on assessment report from SIS Practitioner Consider all other information Use expanded items descriptions to assist in the development of measurable and observable goals Check progress in domains areas by comparing previous SIS scores
