Teacher Evaluationp 27 1 Student needs a teacher report for college application Stamped addressed envelope for each college (2 first class stamps) 4 The Teacher Evaluation/Letter of recommendation is sent directly from CLASSROOM TEACHER to the university 2-3 weeks prior to postmark deadline 2 • Student completes: • Applicant section of the Teacher Evaluation (Demographic information only) • “Requesting a Letter of Recommendation” (pink) form and/or resume • Stamped addressed envelope for each college 3 Submit to your TEACHER the Teacher Evaluation, the “Requesting a Letter of Recommendation” form and/or resume, and the stamped envelope(s) so that a good letter of recommendation can be written. Clearly communicate your university deadline for mailing with the TEACHER. Important: Plan for at least 2-3 weeks for your TEACHER to complete your teacher evaluation/ letter of recommendation and mail it to the university

Arrange a date for evaluation pick-up with the ADVISOR Application is sent directly from the counseling office to the university Important: Plan at least 2-3 weeks for your ADVISOR to complete your recommendation and 3 weeks for your COUNSELOR to provide your rankings and to mail your application to the university
