To make a PCO license application you shall fulfill the following scenario: ➢ Be approx 21 years old ➢ Undergo a DBS check ➢ Undergo a medical examination + eye test ➢ Determine sound topographical abilities and skills ➢ Have a capability to speak, read, listen and write in English ➢ If you demand a PCO license application pack, once it comes you would find the following:

  2. ➢ Envelope: It is to send your forms back but further has a handy checklist on the back make sure you have entailed all the rights forms before you pop them in the post. ➢ PHV/203: It is the actual application form. ➢ PHV/204: It is the medical declaration form which needs to be originally completed by you and then fulfilled and stamped by a GP when you have several medical check. We have TFL permitted GP’s and may get your PCO medical check + eye test done on sight. ➢ Payment details: It explains the fees and payment tools and techniques for your application. ➢ Topographical test information sheet: It outlines what you need to do for your topographical test.
