  2. Connecting Our Community New Jersey SHARES (NJS) developed a partnership with Verizon New Jersey to help our neighbors benefit from the Communications Lifeline Program. Lifeline is available to qualified applicants throughout the Verizon service area. NJS and Verizon are committed to improving the quality of life in our communities and keeping our neighbors safe and connected.

Handle all inquires regarding the Lifeline program • providing a single point of contact. • Provide information about the benefits of Lifeline • and participation in other low-income programs. • Provide outreach to community based • organizations and low-income households • statewide. Program Highlights

  4. Direct self-certifiers to the network of 136 NJS • intake sites. • Review applications for self-certifiers and advise • Verizon of customer eligibility. • Identify reasons for failed matches and provide • solutions for customers. Program Highlights

  5. Provide annual recertification services. • Pro-actively call senior self-certifier households to • offer recertification information and solutions. • Contact eligible households not receiving Lifeline. Program Highlights

Having a telephone is within reach. • Here’s How... • Through Link Up America, eligible new customers receive a • 50% discount on the initial phone connection charge, with the • remaining 50% paid in 12 monthly installments. • Through Communications Lifeline, existing Verizon • customers may be eligible to receive discounted local • telephone service. Eligibilty * Medicaid* Food Stamp Program * General Assistance (GA)* Supplemental Security Income (SSI)* Home Energy Assistance Program (HEAP)* Lifeline Utility Credit/Tenants Lifeline Assistance* Pharmaceutical Assistance to the Aged and Disabled (PAAD)* Temporary Assistance to Needy Families/Work First New Jersey OR If you are age 65 or over and are not participating in one of these programs and your income is at or below 150% of the federal poverty income level. You can enrollat any NJS intake agency (136 sites statewide).

  8. Supporting Community Outreach • The Lifeline Brochure is being distributed to all community partners and at community events (English and Spanish). • Targeted radio advertising, press releases, radio and television interviews. • Periodic ads and articles placed in newspapers.

  9. NJS Client Service Center Staff Staff is available to answer calls regarding the Lifeline program, eligibility and enrollment opportunities M-F 9AM-6 PM . Bi-lingual representatives and a language line are available to assist clients.
