  1. JAGRUTI Dementia Care CenterMost people become a little more forgetful as they grow older and find it more difficult to recall facts and faces. But with statistics showing that half the people over age 85 will develop Alzheimer’s disease, it’s only natural that seniors worry about some symptoms, especially if they seem more forgetful than usual. Recognizing this need Jagruti is resolute to provide with complete Dementia Care solution.How to detect Dementia & other memory deficits ?The dilemma for seniors and their caregivers is that there is no clear-cut line between normal memory changes and the very first warning signs of Dementia. There are though some behaviors that, when seen together, might cause concern. A caregiver might become worried if a loved one exhibits a combination of these symptoms:*Misplaces objects*Has difficulty with orientation or spatial relationships*Has trouble driving*Has difficulty following a conversation*Has become more aggressive or irritable*Has difficulty with routine household tasks.*The importance of these symptoms is underscored if they are accompanied by other changes, such as weight loss, loss of appetite, loss of balance and loss of bladder control.

Acceptance:Jagruti’s strives to provide care so that each individual feels valued for who they are, in happy times as well as in challenging times.  It is Jagruti’s goal to provide an atmosphere where each individual feels understood, supported, and secure.   We endeavor to help people relax mentally, emotionally, and physically & help them develop optimum level of support personal independence, health, security, interests, choices, social inclusion, relationships, satisfaction, rights, dignity, and respect.Communication: It is important to observe, acknowledge, and listen to the verbal and non-verbal cues of the people we serve, supporting their communication in the mode, pace, and time frame of their choosing.  In doing so, we seek to understand each individual’s unique thoughts, interests, requests, preferences, and emotional and physical needs by listening to input from them and their family.Respect: We demonstrate respect by the way we speak and listen and attend.  We also show respect by appreciating, supporting and responding positively to each person’s communication style.

Living conditionWe have Private(single) & Semiprivate(Two Sharing) rooms.  The rooms are warm and welcoming.  Mostly we ask people to bring their own personal belongings, which they are  welcome to do as it helps them to feel more at home and more comfortable with familiar things in sight.  Each room comes with a regular bed, but we will provide a hospital bed if a client needs one. All of our bedrooms are located in one wing of the house, making night-time supervision easy to obtain from our awake-night staff.  Each room is easily accessible to our staff so clients can signal for staff assistance. Our rooms have a hand rail the entire length of the hall for stability and safety. Each room has a lovely view as well.Many clients require staff assistance for all bathroom care, so more independent residents can call staff when needed. The bathroom is equipped with a roll-in shower and grab bars, so that we can meet each client’s changing needs.  The toilets will accommodate any type commode chairs.  We advice each client to participate in their own care for as long as possible.The Activity Rooms offer a pleasant place to socialize and share  friendly conversation. Occasionally our residents will choose to watch something on the television together.

  4. Meals: quality food plays a critical role in the quality of life we all experience. At Jagruti all meals and snacks are prepared in our kitchen and are shared in family style with clients and staff around the dining room table.  Snacks, including fresh fruit and vegetables as well as some sweets like cookies or cake are available on request.Transportation: Transportation to activities, medical appointments, community outings or simply a trip to the garden is provided by our trained staff.Activities: We offer a variety of activities tailored around each client’s social, emotional and spiritual needs and interests.  Activities may include listening to music and music therapy, reading, religious activities, nature, bird and wildlife watching from the living room, playing in-house games.  Looking at family photo albums or items in a treasure box can help to trigger wonderful memories.Housekeeping and Laundry : Housekeeping and laundry are included in the inhouse facilities.  

  5. SPECIALIZED CAREHigh Staff Ratio:  Our high staffing ratios and social activities are designed to care for the whole person and to meet each individual’s deep emotional and spiritual needs. We seek to address the unmet needs that can lead to a sense of loneliness and isolation and produce challenging behaviors.  We offer seminars for caregivers and family members, as well as for friends to learn about memory care issues.On Call Medical Care:  Jagruti provides proactive health care management, with in-house RN nursing assessment and care, on-call 24/7. Our RNs and caregivers work closely to provide on call doctor visits, lab tests.  Jagruti also provides transportation and staffing for outside clinic appointments.  We facilitate clear communication between our clients, their care providers and their loved ones to ensure optimal health and continuity of care.End of Life and Hospice Care:  Our goal is to provide dignified, comfortable palliative care to individuals and their families who are going through the end of life process.Respite Care: Providing care for a loved one is the right choice for many families, but it can also be challenging at times.  We are equipped to provide respite stays while the caregivers are away or to help in recover after a health issue.  Respite stays are typically between 2 and 30 days in length.  Respite clients enjoy all of the amenities of long-term clients.
