  5. Transition This is a seamless process. It begins with the very first assessment!

  8. Components of Successful Transitions • Meet legal requirements • Provide service continuity • Reduce family disruptions • Prepare families and children for new placement • Eliminate duplication of testing and planning meetings

  9. So…………………………. What is this Transition Initiative?

Early Intervention Collaboration Goals: • To reduce the stress for families by eliminating duplication of evaluation and paper work • To eliminate unnecessary evaluation and paperwork for Part B and Part C, so that we: • a. Better serve families and children • b. Make the best use of resources

  11. To improve Child Find efforts for Part B and Part C, in order to identify a greater number of children prior to age 3 • To ensure that IEP and services are in place by age 3

Nuts and Bolts Collaboration • Choose reoccurring days for possible evaluations. • EarlySteps Coordinators will send completed Birth-to-Three Developmental History, and Medical (physical, vision, hearing, etc.). • EarlySteps will notify school district if a child appears to have delays in two or more areas of development between ages 0 – 29 months or as specified by Early Steps and School District.

Designated EarlySteps staff will arrange evaluation and notify appropriate person within the school district. • School district will provide one person who could be any of the following: special educator, Educational Diagnostician, SLP, etc… • EarlySteps will provide additional team members • Assessment will be conducted in the child’s natural environment (if at all possible) using a play-based model with a facilitator and recorders.

  14. School district will be responsible for IDEA paperwork. • Eligibility will be determined for Part C and Part B at time of evaluation. • EarlySteps will electronically provide the school district with draft report within designated time period. • District will then add LDE’s components to the draft report within designated time period. Draft will then be transmitted electronically back to EarlySteps.

EarlySteps will complete report for signatures, obtain signatures and the eligibility form. • EarlySteps will give copy of report and eligibility packet to families, as well as keeping a copy for records. • Once eligibility is determined, a decision about the IEP will be made and documented. In most cases, children will have an IFSP developed andbe served by Part C until the age of three years.

  16. Prior to and no later than 2 years 9 months, Early Steps will provide updated information to District, in order to develop an appropriate IEP. • Updated information should include updated IFSP, progress notes from service providers, and reports. • IEP for Initial Placement will be developed, based on the updated IFSP, prior to the child’s third birthday (between 2.9 and 2.11), so that services can begin without delay. • District will invite EarlySteps to the IEP meeting at discretion of the parents.
