Educational Access Scheme (EAS) • Form online • http://www.uac.edu.au/eas/ • Print, complete form • Work out codes • Prepare documents, originals need to be photocopied, sighted and signed by JP • If health care professionals need to complete one section, then make appointment, needs to be completed by Week 7.

  11. Those needing attendance printed, DO NOT approach SASS staff individually to get a printout! Put your name on list outside Maths staff room by Week 6 Friday 22/8. • Counsellor available to check applications and answer questions in Week 7, TBA.

  12. All completed by Week 8 Friday 5/9 • If educational impact statement is needed, applications must be submitted to counsellor, Mrs Bates, Mrs Lawrence, Mrs Chung or another teacher by Week 8 Friday 5/9 • Student’s responsibility to send application.
