Back ground “Look After Your Legs” • Recognition of inconsistent care delivery/skill levels in leg ulcer management, within Primary Care • Lack of accommodation/time within practice to see patients • Creation of leg shaped information leaflet and patient ambassador role • Moving patient care from a non-clinical setting, closer to home • Setting up the weekly “ Look After Your Legs” clinic 2003 • Patient and public involvement with long term condition management. • Patient empowerment via monthly support group
