  2. Coloration de gram modifiée Legionellose Agent causal Legionella pneumophila : bacille intracellulaire mobile (Gram négatif)

  3. Habitat - réservoir d’eau douce : circuit de distribution d’eau potable surtout chaude (chauffe-eau, pommeaux de douche), systèmes de climatisation d’air, piscines d’eau chaude.

  4. Transmission - par aérosol : la porte d ’entrée est pulmonaire. Rq : il n ’y a pas de contagion inter-humaine ! Facteurs de risques : - après 40 ans, - tabac, alcool - immunodépression

  5. Clinique - pneumopathie aiguë : début brutal, fièvre élevée, toux sèche, AEG, vomissements, tr. psychiatriques.  complications : - détresse respiratoire - insuffisance rénale.

  6. Diagnostic 1. Direct - prel : LBA (liq. broncho-alvéolaire), liq. pleural, hémoculture, urines, biopsie, crachat  frottis : IFD (ou colo Gram modifiée !!)  culture lente (2 à 15j) sur milieu spécial (BCYE) - dans les urines : antigène soluble de Legionella pneumophila sérotype I +++ 2. Indirect : sérologie +++(Ac apparaissent 1 sem ap le début de la maladie, pic à 3-4 sem, disparaissent en 2 à 3 mois)

  8. Antigène soluble urinaire

  9. Encemensement sur milieu BCYE

  10. Traitement -antibiothérapie : macrolides Prévention - nettoyage et désinfection des circuits d’eau, porter l ’eau à 60 °C au moins ou maintenir un taux de chlore de 2 à 5 ppm.
