Turning 65 and Medicare eligibility • If you worked more than 10 years (40 SS quarters) OR • Before age 65 if: • Disabled and have collected SS disability for 24 months. • Have permanent kidney failure (end-stage renal disease). • Have ALS (Lou Gehrig’s Disease) and have collected SS disability for 5 months.

  3. Turning 65 and Medicare eligibility (continued) • If already drawing SS benefits, you will automatically receive a Medicare card about three months before your birthday month. • For both Part A and Part B to begin, do nothing. • To waive Part B, return the enclosed card to SS. • If not yet drawing SS benefits and want Medicare to start at age 65, you must enroll in Part A and Part B through SS. • Can enroll online at www.ssa.gov, over the phone at 1-800-772-1213, or in-person at a local SS office up to three months before the 65th birthday month.

  4. Medicare plan options • Original Medicare (Part A and Part B) covers about 50% of your overall health care costs. • To help cover the remaining costs, there are two options: • Enroll in a plan that pays second after Medicare pays first (a Medigap plan – i.e., a supplement). • Enroll in a plan that contracts with Medicare to administer your Medicare benefits (a Medicare Advantage plan).

  5. UCare 2013 Premium Rates • High Option: $246 per member per month – coverage through the ‘donut hole’ • Low Option: $144 per member per month Two enrollment forms are included in each packet. Additional applications are provided if you wish to enroll in the UCare Comprehensive Dental $21/mo. Mail completed forms to UCare in the return envelope provided. Keep the yellow copy of the form for your records. Spouse may also join UCare upon becoming Medicare-eligible.
