  9. Laboratory Hygiene Review • Does the lab or storage area have an unkempt appearance? • Is there a protocol for lab equipment cleaning and maintenance? • Are tables, equipment, etc. disinfected according to established protocol?

  10. Laboratory Hygiene Review • Are chemicals, equipment, and other materials kept in secured conditions? • Is storage random with no restrictions on where chemicals are placed? • Are acids stored next to bases?

  11. Laboratory Hygiene Review • Are oxidizers next to flammables? • Are water reactives next to the sink? • Are poisons next to a desk?

  12. Laboratory Hygiene Review • Has a hazard analysis been conducted and kept current? • Has a CIL been completed, submitted, and kept up-to-date? • Is the CHP reviewed and evaluated annually?

  13. Laboratory Hygiene Review • Is the CHP available and accessible? • Have all employees been trained on the details of the CHP? • Are there uniform lab hazard signs at each entrance? • Are all containers properly labeled?

  16. Laboratory Hygiene Review • Is storage alphabetical where incompatible substances can come into contact with others? • Are chemicals left where last used? • Are chemicals stored inside vent hoods? • Are flammables stored inside domestic refrigerators?

  17. Laboratory Hygiene Review • Is food stored beside chemicals inside refrigerators? • Are containers, especially glass, stacked on top of one another? • Are chemicals ever stored on the floor? • Are shelves overcrowded?

  18. Laboratory Hygiene Review • Is a carrying container used when transporting chemicals by hand? • Are containers free of evidence of leakage? • Are there any unknown chemicals in the lab?

  19. Laboratory Hygiene Review • Is the sink drain ever used to discard unused chemicals? • Are solid waste materials properly discarded? • Are glass containers touching others on the shelves?

  20. Laboratory Hygiene Review • Must containers be moved to get the one wanted? • Is the shelving strong enough to support the materials on them? • Are shelving secure so there is no chance of tipping?

  21. Laboratory Hygiene Review • Is each shelf fitted with a small lip to prevent containers from sliding off? • Are liquids or corrosive materials stored on shelves above eye level? • Are any containers allowed to hang over the edge of shelves?

  25. Laboratory Hygiene Review • Are aisles clear and unobstructed? • Is there a spill kit in the lab? • Is a hazardous waste collection system used? • Are employees and students trained in hazardous waste collection?

  27. Laboratory Hygiene Review • Was it inspected within the last six months? • Is it clearly visible and accessible? • Is it provided with a modesty curtain?

  32. Laboratory Hygiene Review • Is color-coding practiced? • Are shelf edges painted to match color-coding of labels? • Are shelves labeled to match the chemicals stored there?

  33. Laboratory Hygiene Review • Are chemicals inventoried monthly? • Is the CIL forwarded to Safety and Security? • Are minimum quantities storage requirements maintained?

  34. Laboratory Hygiene Review • Are containers inspected regularly for leakage, color changes, cloudiness, pressure buildup, etc. • Are damaged containers immediately taken out of service? • If so, what happens to the chemical?

  35. Laboratory Hygiene Review • Is maintenance and upkeep a high priority? • Is the storage area and work area well ventilated? • Is air re-circulated at least six times per hour?

  36. Laboratory Hygiene Review • Is the temperature and humidity maintained at a proper level? • Are proper fire extinguishers available and in proper working order? • Are chemicals stored in sunlight, near steam pipes, lab ovens, or other heat sources?
