KEY PROBLEMS The cleanliness of Hospital is an important component in the provision of clean safe care. Thousands of patients die each year from the deadly infections in unclean hospital wards. The hospital-acquired infection situation in Australia is no better than England and may even be worse. Traditional loop mop and bucket have many risks. The quality of cleaning water in the bucket is a big infection control hazard. Staff changes cleaning water after cleaning of 4-6 patient rooms which results in poor cleaning standards in the health care facilities Thus there was a need to find a unique cleaning system which must meet the NSW Cleaning, Environmental and infection standards
AIM OF THIS INNOVATION • This study was planned to investigate that existing cleaning system in hospital can meet required cleaning standards to reduce infection in hospital • Determine if resources are best utilised to reduce infections in hospitals • Determine if cleaning is effective and staff are aware of NSW Cleaning and Environmental standards. • Find a cost effective way of cleaning which reduces WHS hazards, chemical hazards and environmental hazards. • Develop tools for monitoring cleaning standards in hospital • Develop cleaning manual, cheek sheet and record keeping for daily/ weekly/ monthly and annual audits
BASELINE DATA • In Australia it costs almost $10 billion on health care annually due to polluted air inside the building then the air outside (CSIRO2000). • Hospital have closed environment, which is full of air born bacteria and viruses, which results in poor patient & staff health care. • Clair & Linda (2009) reported that there are new fears of deadly superbug outbreaks as cleaning budget are slashed across NSW, which already has Australia’s highest rate of hospital acquired infections • In 1997 a medical journal reported that patients 'have more than a 1-in-10 chance of picking up infections in hospital
KEY CHANGES IMPLEMENTED • Reduced most of WHS hazards related to cleaning services (no accident last three years) • Removed loop mop and bucket cleaning equipment and introduced microfiber mop system • Introduced pre-soaking mop technique to improve environmental standards • Reduced chemical, labour and overhead charges costs • Introduced local mop laundering system to reduce cleaning equipment cost • Improved Infection controls measures by using colour coded mops and cloths. Each mop & cloth were marked with wards names to reduce cross infection from one ward to another ward.
KEY CHANGES IMPLEMENTED • Introduced cleaning monitoring system on daily, monthly basis. • Hospital was divided into different areas such as high, medium, low risks and non-critical areas and developed cleaning services accordingly • Introduced staff training and education program on different types of cleaning, infection control, WHS – manual handling • Staff incentive on compliance of Cleaning, Environmental and infection control standards – Employee of the month
OUTCOMES SO FAR Results are based on 2 years data: • Reduction of microbial count – 96% • Reduced equipment cost (mops) – 50% • Improved staff agronomical activities – 80% • Improved floor drying time – 40% • Chemical saving on average – 94% • Improved water usage – 96% • Improved staff efficiency – 40 % • Improved water disposal – 100 % • Increased laundry cost – 110%
LESSONS LEARNT • Most of the state’s hospitals have difficulty in achieving the required cleaning standards due to non consistent cleaning practices and products. • Cleaning staff are not trained in Infection control, cleaning and environmental standards. Develop cleaning task sheets / pictorial manual to educate staff. • Cleaning staff should have enough cleaning equipments and other resources to carry out duties. • Thus there is a great need to implement the Green Cleaning Program within the healthcare facilities, as it has proven benefits including minimising infection risks, reducing chemical and water usage as well as costs associated with WHS related issues.
