  2. Who Participates in THE DIPLOMA ON! PROJECT? • Began in April 2012 with Brooklyn Center and St. Louis Park. • 2012-2013 SY, Hopkins and Osseo districts joined the project. • 2013-2014 SY, Eden Prairie, Mound Westonka, Richfield, Robbinsdale, and Wayzata joined the project. We also added a collaboration with MVNA (MN Visiting Nurses Association), The Front Door, and Bloomington Public Health. • Diploma On! Specialist is financed through LCTS funds.

  3. What is THE DIPLOMA ON! PROJECT? • Diploma On! Specialist works with identified staff person(s) in each district on their 15-day drop list to generate referrals. • Diploma On! Specialist attempts to locate and contact students in the community via multiple methods: phone, Facebook, school resource officer, friends, and family. • If and when contact is made, the goal is to begin building a relationship and reducing barriers for the student and family in order to re-enroll into a program that fits their needs.

  4. More Information • Goal: Reduce barriers such as mental and chemical healthconcerns, lack of transportation, family stressors, medical needs, past experiences within the school system, etc. • Once a “next program” is identified (home district, area learning center, online, special education program, General Education Diploma (GED), Adult Basic Education (ABE), etc.), Diploma On! Specialist works with student and family to navigate systems in order to get student re-enrolled.

  5. MORE INFORMATION • If and when a student is re-enrolled, the Diploma On! Specialist will see the student at school or in the community 1 to 4 times a month, depending on needs. • Diploma On! Specialist will try to ensure that the student can identify and build relationships in the new program in which he/she is enrolled.

  6. Referrals and Trends to date: • 158 referrals as of 01/15/2014 • 35% Black/African American, 28% Latino, 17% Caucasian • 16% identified special education students • 55% Male and 45% Female • 40% (n= 63) No contact/Declined Services/Moved Out of Area/Re-Enrolled Prior to Intervention

  7. Referrals and Trends to date: • Trends seen in 111 referrals as of 1/15/14 • 45% identified Mental/Chemical Health, 34% Transportation, 12% Unstable Housing, 12% Teen Parent. • 4 students have graduated!

  8. MVNA Update • 19 referrals since project began in late October 2013 • Referral Sources: - 17 referrals from MVNA • - 1   referral from BPH (Bloomington Public Health) • - 1   referral from a sibling • 28 infants and toddlers

  9. MVNA Update • Outcomes to date: • N= 1 Graduated with GED • N= 3 Enrolled in ALC • N= 4 Pending admission ALC • N= 1 Connected with mental health assessment • N= 10 No contact made or need ROI

  10. MVNA Update • Identified Barriers for MVNA Clients • - Transportation • - Daycare (girls prefer ELC location) • - Locating credit status • - Special Ed or General Ed • - Release of Information (locating guardian) • - Homeless
