Student Background First name: Ryan Age: 8 years old Grade: 3rd Classroom Placement: Ryan does not participate in the general education setting during english, math, speech, and occupational therapy. While he is in the general education classroom for social studies and science, Ryan requires an aide support when he is expected to read and write. Reasons student was selected for tutoring- General/special classroom performance: Ryan demonstrates weak auditory skills, delayed receptive and expressive skills, as well as fine motor and visual motor integration skills.

Running Reading Record Ryan's’ Strengths Ryan’s Weaknesses Reading fluency Reading accuracy Demonstrates expression/prosody • Comprehension • Retelling main events • Generating a summary in appropriate sequential form according to the text • Identifying all characters/events • Interpreting text

Sight Words Assessment Ryan read all of the sight words included in the assessment accurately He read the list at a very impressive rate It is obvious that Ryan feels confident with reading 3rd grade level sight words Room for Improvement: Ryan must expand his sight word vocabulary by becoming familiar with higher grade-level sight words

Literacy Interview Ryan’s Responses Possible Reasons Ryan does not like to read Ryan does not like when he has to read at school • Ryan may not find enjoyment in reading due to his home-life situation. • Nobody reads to him at home • Ryan has not witnessed his role-models (family) reading for pleasure • Ryan gets impatient when his peers read word-by-word. He wants to pronounce the word for them so they can read faster.
