  2. Review: • Set Priorities • Drill down, identify root causes • Set Goals • Set Objectives • Identify outcome measures • Review evidence-based intervention strategies

  3. Homework: • Write goals for each priority area (Worksheet #4) • Write objectives for each goal (worksheet #5 & #4) • Identify outcome measures for each objective (worksheet #5 & #4) • Choose Intervention Strategies (Worksheet #7 & #4)

  4. If you have completed a CHA: • Use your selected priorities • If you haven’t selected priorities, select them (if possible)

  5. If you have NOT completed a CHA: • Complete the activities using • Someone else’s priorities • OR ‘made-up’ priorities- but make them as realistic as possible

  6. Key Points • Think of health broadly • Involve community partners throughout • Iterative – not a one-time process • The process is as important as the outcome
