  1. Cost of Coverage* – Standard Premium costs (Bi-Weekly) • * Goal Achievement Premium Discount! • Employee and/or Spouse receive a $10 monthly premium discount (maximum potential monthly discount - $20 for Employee and Spouse) off of the Standard Premium Rate amount if: • ALL 3 Wellness Participation Requirements are completed • A non-tobacco user • In a normal BMI during your upcoming screening • Wellness Goals are met (from the previous year’s screening) Note:Domestic Partners may be enrolled on the plan if part of a registered same sex Domestic Partnership or an opposite sex Domestic Partnership when at least one partner is 62 or older. Domestic Partner contributions cannot be deducted on a pre-tax basis, and Kadlec contributions to Domestic Partner premiums are considered taxable income to the employee. * Coverage of enrolled children will cease at age 26 (unless applicable law requires us to offer coverage for a longer period of time).
