Section Q Referrals • Memorandum Of Understanding between EOHHS and DPH to obtain MDS 3.0 database • Number of individuals for an annual period, who received an MDS 3.0 Assessment, who: • Answered question Q0500B1 • Were age 22 at the time of assessment • Were in a facility at least 90 days • Had an ABI-related diagnosis • Are unduplicated respondents

Transition Entity Contracts • Transition Entity Contracts(Current Contracts end on 06/30/15) • Re-contracting Period: 07/01/15 thru 06/30/17 • MDS 3.0 Section Q & ABI referrals • Housing-Related Activities • Reporting Requirements • Payment and Financial Provisions – Revised: • Enrollee Bundle Payment – under new contract = $1,900 • Participant Bundle Payment – under new contract = $1,900 • Can submit claim 30 days post discharge when Participant maintains community tenure.

Transition Entity Performance Evaluations • MFP Project Office is conducting Performance Evaluations to assess each TEs’ compliance with the terms of the Contract for MFP Transition Coordination Services • The Review measures TE compliance in following areas: • Engagement (Outreach), Eligibility/Enrollment, and Demonstration Transition Coordination, including QoL Administration; • Staffing, Orientation/Training, and Quality Assurance; • IT/Data Systems; • Case Records; and • Reporting • Types of Performance Evaluations • Desk Audit • Site Visit • Results will help TEsclarify program objectives, assess effectiveness and make informed improvements where needed

MFP Sustainability Plan • Massachusetts MFP Sustainability Plan was submitted to CMS on 04/23/15 • MFP Sustainability Plan Highlights: • Utilized input from • MFP Informational Stakeholder Meetings • Regional Round-Table Discussions • Web-based Transition Entity Survey • MFP Consumers • MFP Quality of Life (QoL) Survey Reports

MFP Sustainability Plan (cont.) • Consistent with the ADRC 5-Year Strategic Plan, Massachusetts will leverage its ADRC network to continue the work of transitioning individuals • 8 of the state’s 10 HCBS waivers, along with State Plan Community Long Term Services and Supports, remain Massachusetts’ core approach to sustaining members in the community who have transitioned under the Demonstration • Co-locate MassHealth Eligibility Specialist in key ARDC agencies (BIP Grant) • When complex MassHealth eligibility situations between facility & community occur, MFP Project Office will provide guidance; • CMS Approval of Massachusetts MFP Sustainability Plan is expected by 08/01/15

  25. 811 PRA Eligibility Requirements • All applicants must be under the age of 62 and meet income requirements. • Closed referral process • Priority Categories: • Priority I: Enrolled in Money Follows the Person (MFP) Demonstration and currently in a qualified facility • Priority II: Residing in a long term care facility and eligible for a MassHealth 1915(c) Home and Community Based Services (HCBS) waiver • Priority III: Residing in a long term care facility and not eligible for either MFP Demonstration or a HCBS waiver • Priority IV: Living in the community and eligible for a HCBS waiver

Current & Future 811-Related Housing Resources • Applicants will have the choice to apply for each of the available housing options (811 PRA, PBV, MRVP, and HCVP). • HUD: • 190 811 Project Rental Assistance Units – subsidy attached to project-based units under contract for 30 years • DHCD • 25 Massachusetts Rental Voucher Program Vouchers • 13 Mobile Housing Choice Voucher Program vouchers • 12 Project-based Section 8 vouchers • All applicants must meet program eligibility requirements in order to receive assistance in one of these housing options.
