To Date: • Survey of how FISH is currently done at the institution, and what resources are in place • Pilot 1: 10 cases circulated for FISH, however it is currently done • Pilot 2: 2 cases circulated to try ND-FISH
Survey • Variability in • Equipment • Processing • Scoring strategy • Everyone seeks to score 200 interphase nuclei per hybridization • Cutoffs for normal differ across institutions
Pilot 1 • FISH as standardly done in that lab • 10 cases • 4 slides for each case • 2 pre-etched 15 mm circles so 2 FISH assays per slide • One control sample included (normal donor) • Efficacy and accuracy • 5 V-FISH (Vysis), 1 ND-FISH
Observations—Pilot 1 • All sites were quite good • Missed anomalies (won’t find if don’t look) • Pt 8 had 65% 6q- • ND-FISH distinguished -11 and 11q- in pt 2, and +MDM2 in pt 3 • Scoring inconsistencies • Aqua • Number of cells scored
Summary—Pilot 1 • Excellent results across the board • Identify issues for further discussion and piloting
Pilot 2 • Gain experience with ND-FISH (Mayo std) • Can distinguish monosomy from deletion on ch 17 and 11 • Includes • 6q- • +MDM2 • 13q-x1, 13q-x2, +13q34 • IgH abn
Summary • First experience with ND-FISH went exceptionally well • Need more practice on monosomy vs deletion • Need more accuracy to address prognostic significance • More variability if percentage mid-range • Need to address if % abn will be important • Development of metaphase FISH in CLL
