3. Maine State law • The operators of any facility where any extremely hazardous substance is present in a quantity above the threshold planning quantity are subject to the following: Written plans Annual test Emergency response equipment

4. Written Plan • Identify the facility emergency coordinator and that person's alternate Who is tasked with developing the plan and providing assistance to the LEPC regarding the plan Who is the alternate Are there work and 24 hr phone numbers available

Written plan • Describe the emergency warning systems and list nearby emergency and health personnel What systems are in place to warn employees of a danger List of EMS/Fire/Police who provide coverage along with numbers other then 911 Are there on site medical persons What hospitals/medical centers may be used to treat employees who may be injured

6. Written Plans • Describe employee training and testing programs What level of training do employees receive How often How are they tested and how/where is it documented

7. Written Plans • Describe available response equipment and protective garments What response equipment is on site Where is it located What PPE is available Where will additional equipment and PPE come from

Written Plan • Describe emergency health treatment procedures including notification, facility evacuation and community evacuation and shelter-in-place procedures Emergency health treatment = MSDS Notification of event = AR-1 form Facility evacuation = Existing fire evacuation plan Community evacuation / Shelter in Place = Existing County EMA/LEPC plans

9. Written Plans • Identify transportation routes and transportation methods for extremely hazardous substances How does the material get to you What mode(s) of transportation are used You already have this information from Tier 2 reporting

10. Written Plans • List the names of all companies providing sudden and non sudden accidental coverage to the facility Who provides insurance coverage Who provides spill clean up Phone numbers and Points of Contact

Written Plans • List any mutual aid agreements between the facility and emergency responders or public safety agencies Do you have agreements in place Does the agreement specify who will provide what service If no agreement are your local responders trained to deal with an emergency at your facility

12. Annual Test • An annual test demonstration and annual review must be held by the facility owners or operators to ensure public health and safety Document results Who was involved Any lessons learned

13. Emergency response equipment • The operators of facilities covered under this subchapter shall, through mutual aid agreements with a committee or local emergency response officials, provide for use by emergency personnel, primary response equipment which shall at a minimum include protective clothing and breathing apparatus necessary to contain or extinguish releases of substances handled by their facility
