INCIDENT: Vehicle Accident resulting in injuries to/ or caused by Coalition Personnel Vehicle Accident IMMEDIATE ACTIONS BY UNIT (1) Establishes FP measures -Secures local area -Assesses the situation (2) If injuries involved begin first aid, submit 9 line MEDEVAC request see ‘MEDEVAC Drill’ (3) Gather information on personnel and vehicles involved and draw sketch of accident (digital photos) (4) Establish TCPs to control traffic in the area (5) If HAZMAT involved ‘see HAZMAT Drill’ (6) Conduct recovery of vehicles and equipment (7) Track what hospitals or Aid Stations the injured personnel are sent to (8) Notify Higher HQs FLASH!!! 1. Unit involved secures site, notifies RT TOC Notification Tree: (1) S3 (2) BN CDR (3) BN CSM (4) BN XO (5) GS MP PLT LDR & IZ Police 2. Battle Captain begins notification of appropriate personnel: 3. (3) Obtain name, rank, SSN, DOB, nationality, address, and position in vehicle of all personnel involved (4) Obtain vehicle information on all involved A: Activity (1) What was the cause of the accident (2) Damage to vehicles L: Location (1) Location of the accident (8 Digit Grid) T: Time (1) Time the accident occurred?   13. Discovery of Minefield / UXO INCIDENT: DISCOVERY OF NEW MINEFIELD / UXO IMMEDIATE ACTIONS BY UNIT: (1) Unit secures site. (Digital Photos) (2) Unit DOES NOT handle UXO or mines. (3) Mark the site with a highly visible material: (a) Engineer tape with mine signs (b) Barbed wire or concertina wire (if available) (c) Colored plastic tape (d) 100 mph tape (4) Limit vehicle and pedestrian traffic in the area. (5) Warn local population through interpreters. DO NOT promise the local population a time for UXO removal. (5) Unit informs higher. Immediately Notify (1) All TF Units (2) TF EN 1. Unit secures site 2. Unit reports to TF TOC IMMEDIATE ACTIONS BY TF EN/TF TOC: (1) Plots mine on map. (2) Updates data base on computer. (3) Enter data into Daily Mine Update. (4) Assign clearance to either EOD or Engineer Unit (if necessary). (5) Submit follow-up report. 3. Battle CPT notifies appropriate personnel: Mine/UXO 9-Line Report to 2 BCT TOC: (1) DTG Discovered. INCIDENT: Catastrophic Attack Against HVA (VBIED) (1 of 2) • IMMEDIATE ACTIONS BY UNIT: • Return fire on attacker to destroy or disable IOT prevent penetration of ECP or initiate device. • Immediately report to TF describing type of attack and identification of any vehicle/s or personnel that carried out the attack. • Immediately close Checkpoints to all vehicle and foot traffic and conduct consolidation and reorganization. • CDR deploys CO/TM QRF to reinforce Checkpoints and security positions • Evacuate areas as required. Only evacuate areas where further attacks may endanger personnel • 6) Conduct damage assessment of impact area and cordon within 500m radius report to TF TOC damage assessment and full salute report and grids/locations of cordon positions. Attack 1. Unit receives attack at HVA. 2. Battle CPT immediately positions inner FOB security personnel to respond to possible enemy penetration of FOB? 3a. Battle CPT directs all units to close Checkpoints to include Entry Control Points to all non-military traffic and sends BOLO for possible vehicles or personnel that may be carrying secondary devices. 3b. Directs selected inner FOB sites to deploy counter-mobility and counter-blast equipment to stop, slow any secondary devices. (Pre-positioned tire rippers, PVAB system, Reposition SEPs, etc.). 3c. TCPs to set cordon and stop all non-essential traffic moving to attacked HVA. 3d. Battle CPT directs QRF1 and Mortar Platoon to go to REDCON 1 and gives a BPT to establish TCPs to set cordon and stop all non-essential traffic moving to attacked HVA or search for suspected secondary devices/personnel. 3e. Directs responsible Company Commander to site (if not already there). SALUTE REPORT to 2BCT TOC: S: Size (1) How large is the explosive device? (2) How large was the explosion? (3) What was the method of attack (VBIED, IED, or combination of, etc.)? (4) Are there injuries (soldiers/civilians)? A: Activity (1) Which unit and element sent the report?
