  2. The Purpose of Nottingham’s ‘Staying in Control’ pilot To practice the extension of self direction into the NHS with people with dementia and their carer.

The ‘Staying in Control’ Pilot Population • Pilot Population – • Be admitted to the intermediate care service, either at home or at the Willows • Have a cognitive impairment (including diagnosed dementia) recognised by the Intermediate Care Team. • Able to live at home following discharge from the intermediate care service • Have a carer

  4. ‘Staying in Control’ Pilot Objectives • To explore patient experience of receiving a personal budget. • To explore carer experience of receiving a personal budget • To explore the impact of patients and carers receiving a personal budget on service providers • To explore the impact of patients and carers receiving a personal budget on the use of health and social care services • To explore the systems and processes that are required to enable Nottingham to provide a personal budget, including healthcare resources, to a service user with dementia and their carer.

  5. Nottingham’s ‘Staying in Control’ Pilot – ‘John’ Nottingham’s draft SAQ has been completed ‘John’s’ Individual Budget = £142 per week

  6. Nottingham’s ‘Staying in Control’ Pilot – ‘John’ Support Planning • Doesn’t like women to help him to have a shower • Likes people to be on time • Doesn’t really like to socialise too much • Used to like going swimming everyday • Used to like Tae Kwon Do

Issues so Far… • Integrating health and social care budgets: • Not many healthcare needs BUT • The Intermediate Care Service is jointly funded • Members of the team thinking in a more person centered way • Highlighted issues regarding PBs • Identified systems and processes required • Peer support network • Got NHS Nottingham City talking about personalisation

  9. Issues so Far.. • Workforce Capacity • Support Planning takes a long time • Current local targets do not allow time for support planning • Hard to recruit to the pilot

  10. General Risks • Financial abuse • Mental capacity • Reduced economies of scale • Service provider regulation • Quality of care • Training of Personal Assistants • Clinical evidence

  11. NHS Personal Health Budget Pilot Site • NHS Nottingham City have been identified as a provisional Department of Health Personal Health Budget Pilot Site • The full project plan will: • Build on the findings of the ‘Staying in Control’ pilot site evaluation. • Focus on long term neurological conditions. • Include service users who are currently receiving a direct payment and become eligible for NHS continuing care.
