  1. Record of Practice Hours and Experience Completing this record Information will appear on each screen explaining how to fill out this section of the portfolio. When you are ready to move on to the next slide, just click your left mouse key or press the enter key or the right arrow key on your keyboard.

  2. Your student should enter their full name, university personal identifier and the course number e.g. KYN107 in these spaces. If this is the first Record of Practice Hours in the portfolio then a 1 should be entered here. Subsequent records should be added chronologically and numbered sequentially: 2, 3, 4, 5 etc.

  4. This is the place to enter the start date of the practice learning period and the end date of the same period.

  6. If this is the first practice learning period, your student should enter the number of practice learning hours completed, under ‘Number of hours’ and the same total under ‘Total hours to date.

  8. For the next practice learning period, your student should enter the number of practice learning hours completed under ‘Number of hours’ and add this to the preceding ‘Total hours to date’. Thus there is an ongoing record of the total hours completed so far. By the end of the course the final number of hours completed must be equal to or exceed that required for the particular practice course.

  10. The next three entries must clearly identify where the learning in practice experience has taken place (e.g. Anywhere Hospital; Somewhere Health Centre), the type of experience (e.g. acute medicine; general practice) and whether or not it is the usual work setting.

  12. During the Common Foundation Programme the student must gain exposure in practice to clients from each of the four branches of nursing. So, during a particular practice learning period, if a student has the opportunity to learn in practice about adult service users and service users with mental health conditions, then these two boxes should be ticked. It is important to ensure that across the practice learning periods your student also has the opportunity to learn about those service users who are children/adolescents or who have a learning disability. Once this exposure has been achieved the relevant box can be ticked on the related Record of Practice Hours sheet.

Similarly, during the Common Foundation Programme your student must also gain experience in a range of settings: one extended complementary experience away from the normal work setting; community-based care; acute patient care; continuing care / rehabilitation. In addition your student should gain experience across the 24 hour / 7 day a week cycle. During each practice learning period all the types of experience that have been gained during that period should be ticked.

  16. At the end of the practice learning period, your student should present you with this sheet, which you should check for accuracy. You should then print your name in the relevant space, enter your designation e.g. Staff Nurse, Community Psychiatric Nurse, Practice Development Nurse and sign and date your entry. Your student should print their name, sign and date the sheet too. The Programme Tutor will complete his/her section of the sheet at the next tripartite discussion meeting. If the Record of Practice Hours sheet relates to the extended period of complementary practice, where your student has been supported by a practice supervisor, then the practice supervisor should complete the relevant sections of the sheet instead.

Ensure your student has entered their name, OU PIN and the course number at the top of this sheet. Write a short summary of your student’s performance highlighting achievements and identifying areas to be developed during the next practice learning period. The practice supervisor should complete this sheet for a complementary practice experience.
