  2. Learning Objectives: • Review the licensure process for the state of Maryland • Understand the licensing process in a stepwise approach • Organize materials and information to allow for a smooth transition from Pharm.D. candidate to licensed pharmacist

Step 1:Gathering the Materials • Obtain the Pharmacy Experience Affidavitfrom Room 224 in Pharmacy Hall • This form fulfills the “College Affidavit” portion of the pharmacist licensure application packet; therefore, it is necessary to obtain this above all else • For those that did not attend, this form was announced by Dr. Morgan during the class meeting in the fall semester

Step 1:Gathering the Materials (cont.) • Using the web browser of your choice navigate to: http://www.dhmh.state.md.us/pharmacyboard/forms/pharmacist.htm • This website contains forms and applications for pharmacists from the Maryland Board of Pharmacy • Download theMaryland License Application under the “Examination Applications” sub-heading

Step 2:Filling Out the Application • Photograph: • Paste any recent photograph. • Shoulder up is preferred. • May be a printed digital photograph, providing good quality • Section 1: Identification • Fill out personal information in the spaces provided • Do not omit any information unless specifically instructed to do so • Section 2: Foreign Pharmacy Graduates Only • The applicant may leave this section blank

Step 2:Filling Out the Application (cont.) • Section 3: All Applicants Complete This Information About Your Pharmacy School • This section is in the format of the Pharmacy Experience Affidavit which may be obtained from PH224, as stated in • The affidavit actually fulfills this section so simply include it when sending the application to the board

Step 2:Filling Out the Application (cont.) • Section 4: Pharmacy Experience • NOT necessary to complete if the applicant is graduating from the University of Maryland School of Pharmacy or Howard University • If the applicant is planning on applying to another board of pharmacy for licensure via the score transfer option on the NAPLEX, this section MAY BE be useful, and SHOULD probably be completed • This section may satisfy other boards’ additional hour requirements. Contact state board(s) to confirm this.

Step 2:Filling Out the Application (cont.) • Section 5: All Applicants Must Answer the Following Questions • This section must be completed by all applicants as directed • Do not omit any information unless specifically instructed to do so • Section(s) 6-14: • Answer these Yes or No questions truthfully and accurately because if they are omitted they could delay the issuance of a license • Do not omit any information unless specifically instructed to do so or it is not necessary

Step 2:Filling Out the Application (cont.) • Section 15: “Necessary Documents” Checklist • Circle the YES or NO options accordingly to each list item to ensure the applicant has included all the required items to complete the application • Notes: • The “Notarized Employer’s Affidavit of Actual Pharmacy Experience” • Complete only if the applicant did not attend the University of Maryland School of Pharmacy or Howard University OR • The applicant is applying to another state board (addressed in section 4)

Step 2:Filling Out the Application (cont.) • The “Letter Indicating hours on file at Board of Pharmacy Office” section can be marked “YES” as you are including the Pharmacy Experience Affidavit (Section 3) • The “Foreign Pharmacy Certificate Original” may be disregarded unless applicant fits the criteria • The “Oral English Competency Examination Results” section may be disregarded as this examination is administered separately and faxed directly to the board by Berlitz

  11. Step 2:Filling Out the Application (cont.) • Section 16: • This section is a personal statement stating the applicant provided true, correct and complete information on this application. • This section MUST be signed

Step 2:Filling Out the Application (cont.) • The Application is Now Completed (TIPS before submitting) • Make sure all the necessary paperwork has been enclosed • Remember to include the Application Fee ($100.00) • Finally, mail the application to the address provided on the application then proceed to Step 3: Applying for the NAPLEX and MPJE • Note: Both Steps 2 & 3 may be completed on the same day.

Step 2:Filling Out the Application (cont.) • After the application has been sent, received and processed, the applicant will receive: • A letter from the Department of Mental Health and Hygiene with a candidate number which is: • Used when the applicant communicates with the board • Used to retrieve NAPLEX, Maryland MPJE and Oral Competency examination scores from the board website

Step 2:Filling Out the Application (cont.) • A separate piece of paper is included with the candidate number letter that indicates information still required before a license is issued: • “Oral Comp. Exam Score” • Applicant may or may not have completed this already depending on date of application • “Graduation Confirmation” • Sent by the School of Pharmacy after 5/19/2006 • “NAPLEX 75+ Score” • “MPJE 75+ Score” NOTE: This letter is NOTtheAuthorization to Test (ATT) letter

  15. Step 3: Applying for the NAPLEX and MPJE • Navigate a web browser to: www.nabp.net • Go to “Examinations” in the left hand columnar menu and select “NAPLEX/MPJE” from the list that drops down once clicked

  16. Step 3: Applying for the NAPLEX and MPJE (cont.) • On the webpage that appears after selecting NAPLEX/MPJE, select the icon that states “Start Exam Registration” at the right of the window as the page is scrolled down

  17. Step 3: Applying for the NAPLEX and MPJE (cont.) • On the next webpage that appears select the state you plan to take the NAPLEX/MPJE in from the available drop down menu, in this case “Maryland” • Note: if you plan to do score transfer, still choose Maryland at this stage of the process.

Step 3: Applying for the NAPLEX and MPJE (cont.) • Once the state is selected, the website will automatically redirect to the appropriate registration page • The registration process is very easy to complete and is also how to pay for the NAPLEX and MPJE as well as Score Transfers • Print and/or save all receipts as no one wants to pay twice for these exam(s)

Step 3: Applying for the NAPLEX and MPJE (cont.) • Once registration is complete, two letters will arrive within a few weeks from “Thomson Prometric,” whom administers the examinations on behalf of NABP • Letter 1: Informing you of your Authorization to Test (ATT) number for the NAPLEX (good for 1 year in M.D., check in other states) • Letter 2: Informing you of your Authorization to Test (ATT) number for the MPJE (good for 1 year in M.D., check in other states) NOTE: Letters may arrive at different times.

Step 4: Registering to Take the NAPLEX/MPJE • Once the Authorization(s) to Test (ATT), eitherone, arrive the applicant may schedule a test date: • Navigate a web browser to: http://www.prometric.com/Default.htm • Select “Schedule a Test” and follow the step-by-step instructions to find and register a: • A testing center • A testing date • NOTE: This process must be completed twice, individually: • Once for the NAPLEX • Once for the MPJE

Step 4: Registering to Take the NAPLEX/MPJE (cont.) • Print and save all “receipts” during both registrations as they contain the “Confirmation Number(s)” which are required for examination • The allotted time for the NAPLEX exam is 5 hours • It is recommended to arrive 30 minutes prior to the start of the exam • The allotted time for the MPJE is 3 hours • It is recommended to arrive 30 minutes prior to the start of the exam

Step 5: The Oral Competency Exam • The school will announce dates that Berlitz will be at the school to administer the exam • The exam is required to be licensed in the state of Maryland • The exam may be taken at the applicant’s leisure • If an applicant chooses to take the exam on a date other than that provided by the school they may contact Berlitz directly to set up an appointment • Testing centers and phone numbers may be found at: http://www.dhmh.state.md.us/pharmacyboard/license/BERLITZ.htm

Step 6: Checking Scores • To check scores for the NAPLEX, Maryland MPJE and/or Oral Competency Exam the applicant must have their candidate number from the Maryland Board of Pharmacy • Scores may be checked at: http://www.dhmh.state.md.us/pharmacyboard/license/pharm/scores/index.htm • Scores are good for 1 year in Maryland; however, other states may vary • e.g. Pennsylvania only recognizes NAPLEX scores for 6 months
