  2. Today’s objectives • You will learn how to: • Explain the purpose of a Patient Navigator in the SCH model. • Name three ways in which the role of the Patient Navigator differs from that of the Care Coordinator, the Interpreter and the Social Worker.

  3. How do Patient Navigators help? • Establishing trust and rapport with patients in their own language • Assuring that families understand their children’s diagnosis, the care being delivered, how to ask questions and advocate for their children • Teaching families how to successfully navigate the healthcare system on their own • Assuring that providers understand the families’ cultural needs

  4. Comparison to an Interpreter Interpreter Patient Navigator Explains concepts Acts as a Cultural Mediator Establishes relationships with family and staff Educates family and staff • Conveys message • Acts as Cultural Broker

  5. Comparison to a Social Worker Social Worker Patient Navigator Establish trust & rapport with the family. Establishes relationship. Connects the family to those resources. • Assesses family needs. • Provides psychosocial support. • Provides information about resources.

  6. Comparison to a Care Coordinator Care Coordinator Patient Navigator Educates families how to use the services. Follows up to assure that care is occurring as planned. Evaluates appropriateness and coverage of support. Assures the family can use the resources. • Arranges access to services. • Promotes continuity of care in the community. • Supports family during hospitalization and transition to home. • Connects family to resources.

  7. Critical difference with all three • Patient Navigation is acustomized service focusing on teaching the family so that they can manage their child’s care • independently.
