Knowledge Gaps and Research Opportunities • What we know about the clinical epidemiology of stroke • Large vs small vessel disease • Differences in risk factors, their interactions • Positive protective factors • Consider vascular system as an “organ” • Understand the role of HIF, NO, and EPO as protective factors in the brain

Knowledge Gaps and Research Opportunities (2) • Genetic methods to look at stroke-related genes and polymorphisms in large vs small vessel strokes • Animal models for stroke, particularly small vessel disease. • Interactions of race/ethnicity and gender for anemia and vascular disease • Risk and protective factors • Across aging transitions (e.g., hormonal changes)

Knowledge Gaps and Research Opportunities (3) • Mechanisms through which HIF effects cognition, affect, and mobility • Effects of the Renin Angiotensin System (RAS) on cognition, affect and muscle function • Attention to vascular function instead of morphology • Differential effects of risk factors and treatment on large vessel vs microvascular disease • Effects of stress on hpertension and vascular disease

Knowledge Gaps and Research Opportunities (4) • How to identify/define specific symptoms related to vascular causes of cognitive, affective, and mobility dysfunctions compared to other causes. • Improved methods for imaging brain vasculature, function and control – especially for small vessels. • Tools for measurement of related functions that are compatible with imaging technologies. • Continuous monitoring of multiple aspects of cardiovascular and cerebrovascular function.Attention to vascular function instead of morphology • Intervention studies – incorporate intermediate and distal outcomes measures for TMF. • Role of lipids in cardiovascular function

Research Priorities: Unravel the Role of the Cardiovascular System in TMK • Understand specific phenotypic characteristics aTMFssociated with different manifestations of cerebrovascular disease • Large vs small vessels • Develop tool for continuous and simultaneous monitoring of a variety of physiologic processes that contribute to TMF • How to leverage protective mechanisms such as HIF, NO, or EPO to modulate ischemic damage in organs controlling THF • Animal models to study small vessel disease (e.g., NOTCH3)

  7. Research Priorities • Examine mechanisms through which cardiac diseases and inteventions influence cebrovascular and muscular function • Valvular disease • Heart failure • Atrial Fibrillation • Post-pump syndrome
