  2. Claims Process • Injured worker reports an incident and/or injury. • Visits GP – GP issues workers compensation prescribed medical certificate.

  3. Claims Process • Making a workers compensation claim • Worker must have the prescribed medical certificate before being able to complete a claim. • A worker should not be advised to complete a claim form.

Claims Process • Completing a claim form • Only the worker can complete a claim – it is not to be completed on their behalf. • The worker completes the left hand side – Injured workers details only (Q1-35) • The Employer’s details are completed in the IMU (Q36 to 66) • A claim is not a valid claim unless a workers compensation claim form is accompanied with a prescribed medical certificate – so do not send one without the other.

Common questions • What happens at age 65 or above? • Workers compensation weekly payments cease on 65thbirthday • If claim made at age 64 or greater, then entitlement is for 12 months only • Who processes wages whilst worker is injured? • The Injury Management Unit, no ESS bookings are necessary or completion of leave forms. • How long does a worker have to complete claim from injury? • 6 months from date of injury/incident

Common questions • What happens to wages whilst a claim is being investigated? • A worker is entitled to receive wages for the period of the investigation (up to 84 days) and; • Medical expenses paid up to $5000.00. • If the claim is disputed, wages and benefits cease. No repayment is required.
