  2. Epidémiologie Les sténoses carotidiennes : • 5 à 10 % des sujets de plus de 65 ans • 20 à 30 % des patients coronariens ou artéritiques • 45 000 Accidents ischémiques cérébraux • 80 % : origine carotidienne • 2/3 carotide extra-crânienne

  3. Risque évolutif • Sténose 50 à 70 % : 7 à 11 % d ’AVC à 5 ans • Sténose > 70 % : 17 à 19 % d ’AVC à 5 ans • Sténose > 90 % : 64 % d ’AVC à 2 ans le traitement chirurgical réduit le risque à : 3 à 5 % à 5 ans

  4. Indication Chirurgicale • Sténose serrée • soit 50 % selon étude NASCET • ou 70 % selon étude ECST

  5. Indication Chirurgicale • Sténose symptomatique • AIT : accident ischémique transitoire • AVC : accident vasculaire constitué • respect d ’un délai de 3 semaines • œdème • infarctus hémorragique

  6. Indication Chirurgicale • Critères individuels : • risque hémodynamique ( occlusion controlatérale) • risque embolique ( instabilité de la plaque, lésion ulcérée ) • évolutivité de la sténose
