Current Model • Referrals from GP’s and OOH’s to Consultant • Planned admissions from OPD • Choice: Admit or referral on to other community specialist teams eg. behavior support, forensic team, epilepsy nurse specialist. • Common admissions: behavioral problems eg. violence, extreme disruptive behavior, stabilization of medication, breakdown in home circumstances or placement, forensics.
In Development: Crisis Intervention Home Support • Aims: to avoid hospital admissions, to maintain people within their own homes and to expedite discharge. • DHSSPS funded pilot; evaluation – planned evaluation in 6/12 • To provide safe & effective care in home (rapid response) & to intervene in an emergency or crisis which may otherwise have led to an admission. • Home support aims to effectively reduce the time spent in hospital thus improving through-put & reduce the likelihood of readmission. • Revolve around individual client needs as opposed to the system of care • Flexible & responsive • Service 7 days pw / plus on-call
