Frail Older People • 11,005 patients aged 75 and over • 10% of our population • Emergency Admissions in over 75s = 2,348 • 28% of all emergency admissions • Emergency Admissions in over 65s = 3,709 • 44% of all emergency admissions • >75s = 3,898 A&E attendances • >65s = 7,013 A&E attendances
Frail Elderly Session/Primary Care Session • Patients are lonely • Bypass number (1 number/HUB) • Need to be able to share records - EMIS community • Specialist nurses (Integrated/Standards/Falls) • Enhanced Care Team working in a different way • Explore night sitting • More visits/Admissions earlier in the day – Left Shift • Variation/Info (Emergency/OOH/Care homes OOH/Ambulance/ A & E presentation) • Demand on appointments • Patient expectation higher • Multiple number of prescriptions • Only 10 minute consultations – not enough time for complex patients • Worried well – anxiety / isolated/ lonely
