Case Management @ Centennial Case Management Working Group • Students who pose a risk of harm to self or others • Students whose behaviour involves multiple touch points or College areas Case Manager Lead • Students at high risk of attrition due to a mental health or addiction concern • Students experiencing significant mental health or addiction challenges who require care coordination or transitional case management support

Case Manager Lead Supports students who: • Are at risk of (in)voluntary withdrawal due to a mental health or substance use-related concern • Are returning to school following a leave of absence, hospitalization or emergency department visit for a mental health or substance use-related concern • Experienced a mental health or substance use-related crisis on campus that required a mobile crisis, EMS or police response • Attempted suicide • Require community-based mental health and addictions assessment, treatment or support

  6. Meet Rob • Age 18 • First year (2nd semester) student, in good academic standing • Recent relationship breakup • Increase in alcohol use • Current thoughts of suicide • Prior incident in September involving alcohol (completed Check Your Drinking – Residence Life) • History of depression

Crisis Management Weekend/After Hours Response: Residence Life & Security Residence Life Notifications • Emergency contact • Case Manager Lead (follow up with hospital, student) • Residence Outreach Counsellor (follow up with Residents & staff) Life Safety & Security Notifications • Dean of Students • Director of Student Life • Director of Counselling Services • Case Manager Lead

Case Management Case Manager Lead • Case coordination • Comprehensive needs assessment & support coordination • Advocacy Case Management Working Group • Risk assessment & safety planning • Monitoring & accountability for support plan Residence Life • Wellness contract Counselling Services • Outreach counselling

  9. Outcomes for Rob • Hospital outpatient clinic support while waitlisted for community psychiatrist • Transitional case management support pending referral to community-based program • Campus counselling services • Engagement in campus and residence life
