Requestor submits shipment request form on EHS website www.ehs.utah.edu , attaches copy to chemical to be shipped 2 1 EHS bills requesting department back with a Campus Order for shipping expenses 6 Shipping coordinator packages chemical and prepares shipping documentation per applicable regulations 3 Shipping coordinator notifies requestor of shipment completion by email. This will include tracking number. 5 Carrier picks up chemical for shipment from EHS. 4
