Unplanned Event – MOF Propane Leak • February 7, 2012 - Way #4, MOF. • Replacement of a 2 inch SS isolation valve. • Propane leak occurred at valve after line was in service. • Appropriate weld procedure and certifications not provided. • Understand requirements of the job. • Provide documentation required by contract. • Provide qualified personnel for assigned tasks.
