  3. Unplanned Event – Argon Pipe Leak • June 12, 2012 - Southeast corner of Building 213. • Concrete forms were being installed. • Form pin was driven into a 1 ½” copper argon gas pipe. • Ground not marked after backfill. • Follow procedures. Must renew excavation boundaries and marks.

  4. Crane Operations • Problem areas – Crane Related Deficiency Reports (CRDR), lift plans • Accident Rates – 1 accident at Pier 6 since last meeting. Going well. Keep up the good work. • When the cable touched the electrical boom, it was determined that the cable was a part of the crane; therefore, it was interpreted as if the crane hit the electrical boom. • When we write on a lift plan that casual or incidental contact will take place, the crane quality control team must give written approval of the lift plan. Part of crane accident reporting procedure.

  5. Definition of a Reportable Crane Accident • An accident occurs when any one or more of the elements in the envelope of operation fails to perform correctly during the L&H operation, including operation during maintenance or testing resulting in the following: • (a) Personnel injury or death. • (b) Material or equipment damage • (c) Dropped load • (d) Derailment • (e) Two-blocking • (f) Overload • (g) Collision, including unplanned contact between the load, crane and/or other objects.

  6. O27 - Safety Contractor propane cylinders • All contractor propane cylinders shall be identified with a label or tag containing a minimum of company name and emergency telephone number.

  7. O27 - Safety Grating hazard (bicycle hazard) • Grating openings run parallel to the primary direction of travel. Bicycle tires can get stuck causing the bike to come to a sudden stop. • Please reports any such hazards to your contract coordinator.

  9. O27 - Safety September is Fire Prevention Month • 35 foot rule • Reporting fires • Fire aboard USS Miami
